血液透析治疗慢性肾脏病患者的效果及对肾纤维化、肾功能等的影响

         

摘要

目的 观察血液透析治疗慢性肾脏病患者的效果及其对透明质酸(HA)、纤维连接蛋白(FN)、尿素氮(BUN)、血肌酐(SCr)、白蛋白(Alb)等水平的影响.方法 选择2014年10月—2016年12月海南医学院第二附属医院肾内科诊治的慢性肾脏病患者198例作为观察组,同期体检的健康人群120例作为健康对照组.观察组行血液透析治疗,每次透析时间4 h,每周1~3次,连续治疗3个月.比较观察组透析前后HA、C-Ⅳ、PC-Ⅲ、FN、BUN、SCr、Alb水平及肾功能分期变化.结果 观察组HA、C-Ⅳ、PC-Ⅲ水平明显高于健康对照组,FN水平显著低于健康对照组,差异具有统计学意义(t=13.739、27.399、10.969、20.289,P均=0.000);透析后观察组HA、C-Ⅳ、PC-Ⅲ水平明显低于透析前,FN水平显著高于透析前(t=18.628、21.371、22.564、22.640,P均=0.000);观察组BUN、SCr、Alb水平明显低于透析前,差异具有统计学意义(t=18.937、14.953、12.398, P均=0.000);与健康对照组比较,观察组BUN、SCr、Alb水平升高,差异具有统计学意义(t=11.517、4.283、11.973,P均=0.000);透析后慢性肾脏病患者肾功能损伤均得到改善,其中Ⅰ期、Ⅱ期、Ⅲ期患者肾功能损伤改善更为显著,差异具有统计学意义(χ2=17.522、4.000、4.383, P<0.05).结论 血液透析能有效改善慢性肾脏病患者肾纤维化及肾功能指标,降低肾功能分期.%Objective To observe the effect of hemodialysis on chronic kidney disease and its effect on HA, FN, BUN,SCr,Alb. Methods One hundred and ninety-eight cases of patients with chronic kidney disease were selected as stud-y group,which were treated in department of Nephrology of Hainan Medical University The Second Affiliated Hospital from October 2014 to December 2016. 120 cases of the same period of healthy subjects were selected as the control group. The pa-tients of study group were treated with hemodialysis(4h/time,1-3 times per week,continuous treatment for 3 months). The levels of HA,C Ⅳ,PCⅢ,FN,BUN,SCr,Alb and the changes of renal function in the study group were compared before and after dialysis. Results The levels of HA,C Ⅳand PCⅢin the study group were significantly higher than those in the control group,the level of FN was significantly lower than that of the control group (t=13.739,t=27.399,t=10.969,t=20.289,P=0.000);After dialysis,the levels of HA,C Ⅳ and PC Ⅲ in the study group were significantly lower than those before dialysis,and the level of FN was significantly higher than that before dialysis (t=18.628,t=21.371,t=22.564,t=22.640;P=0.000,P=0.000,P=0.000); The levels of BUN, SCr, Alb in the study group were significantly lower than those before dialysis (t=18.937,t=14.953,t=12.398;P=0.000,P=0.000,P=0.000); The levels of BUN, SCr and Alb in the study group were significantly higher than those in the control group (t =11.517,t =4.283,t =11.973,P =0.000);After dialysis,renal dysfunction in patients with chronic kidney disease were improved, in which, stage Ⅱ and Ⅲpatients improved more significantly (χ2=17.522,χ2= 4.000,χ2=4.383;P<0.05). Conclusion Hemodialysis in the treatment of chronic kidney disease can effectively improve the renal fiber and renal function indicators, improve the renal function staging.
