Objective To compare the clinical effects of InterTan and PFNA Ⅱ in the treatment of intertrochanteric fracture in elderly patients.Methods Elderly patients (more than 80 years old) with intertrochanteric fracture from January 2012 to January 2016 were retrospectively analyzed.A total number of 42 patients were collected who underwent either InterTan or PFNA Ⅱ fixation.The perioperative information was recorded.The complications, healing time and Harris Hip Score were compared between two groups.Results The average operating time (P <0.05) and the average volume of intraoperative bleeding (P <0.05) were longer or greater in InterTan group than PFNA group (t =2.331, t =3.948, P =0.035, P=0.016).There were 5 (25.0%) internal fixation related complications in the InterTan group, while PFNA Ⅱ experienced 6 Lateral greater trochanter fractures (27.3%), the difference was not significant.The average follow up period was 6.8 months.The average duration to bony union in InterTan group was (14.0±3.2 weeks), which was shorter compared to (14.5±3.3 weeks) in PFNA Ⅱ group, however, the difference was not significant.There was no significant difference of statistics in excellent rate between two groups during the last follow up (P >0.05).Conclusion Both InterTAN and PFNA Ⅱ can be used in the intertrochanteric fractures in the elderly patients to support early functional exercise.However, PFNA Ⅱ had advantages of less operation time and blood loss.%目的 比较InterTan和PFNA-Ⅱ髓内固定系统治疗高龄股骨转子间骨折的临床疗效.方法 回顾性分析2012年1月-2016年1月武汉大学人民医院骨一科采用InterTan(InterTan组, n =20)和PFNA-Ⅱ(PFNA-Ⅱ组, n =22)治疗的高龄(≥80岁)股骨转子间骨折患者的病例资料,对比分析2组患者围手术期资料、术后并发症、骨折愈合时间、髋关节Harris评分等情况.结果 InterTan组手术时间、术中失血量显著多于PFNA-Ⅱ组(t =2.331、3.948, P =0.035、0.016);InterTan组局部及全身并发症7例(35.0%),置入物相关并发症发生5例(25.0%),PFNA-Ⅱ组分别为8例(36.4%)和6例(27.3%),2组比较差异无统计学意义( P >0.05);所有患者平均随访3~12个月,中位数6.8月, InterTan组平均骨折愈合时间为(14.0±3.2)周,稍短于PFNA-Ⅱ组的(14.5±3.3)周,但差异无统计学意义( P >0.05);2组末次随访时髋关节Harris评分比较差异无统计学意义( P >0.05).结论 InterTan和PFNA-Ⅱ髓内固定系统均可用于治疗高龄股骨转子间骨折,可获得即时稳定性,进行早期功能锻炼.但PFNA-Ⅱ髓内固定系统具有手术时间短、术中失血量少等的优势.
