Objective To investigate the efficacy and safety of extracorporeal shock wave therapy (ESWT) for calcifying tendinitis of rotator cuff with the method of meta-analysis. Method The randomized controlled trials of ESWT for calcifying tendinitis of rotator cuff were searched from PubMed, Springer Link, CNKI and other databases. The related datas were selected in standard of randomized controlled trials, interventions and other inclusion criteria; the literatures that did not meet the inclusion criteria were ruled out. The datas were analyzed by Manager Review 5.0. Result Five randomized controlled trials and 302 cases of calcifying tendinitis of rotator cuff were eligible for meta-analysis. The results of meta-analysis showed that the effective rate of treatment group was significantly higher than control group [MD = 21.15, 95%CI (2.84, 157.44), P < 0.01]; the visual analogue scale (VAS) scores of treatment group was significantly lower than control group [MD = -3.37, 95%CI (-5.25,-1.50), P < 0.01]; Constant-Murley score of treatment group was significantly higher than control group [MD =24.09, 95%CI (11.56, 36.62), P < 0.01]; the average particle size of the calcium deposition in treatment group was significantly less than control group [MD = -10.49, 95%CI (-19.28, -1.70), P < 0.01]; the calcification disappearance rate of treatment group was significantly better than control group [MD = 24.24, 95%CI (3.35, 175.20), P < 0.01]. Conclusion ESWT for the calcifying tendinitis of rotator cuff has good clinical effect that include relieving pain, improving shoulder joint function, reducing the average particle size of calcium deposition and improving the calcification rate of disappearance.%目的 采用荟萃分析方法评价体外冲击波疗法(extracorporeal shock wave therapy,ESWT)治疗肩袖钙化性肌腱炎的疗效和安全性.方法 分别在PubMed、Springer Link、CNKI等国内外数据库中检索体外冲击波(extracorporeal shock wave,ESW)治疗肩袖钙化性肌腱炎的病例随机对照试验(randomized control trails,RCTs)的相关文献,严格按照满足RCT、干预措施等纳入标准和不符合纳入条件的排除标准对文献进行筛选并提取相关资料,采用Review Manager 5.0软件对资料进行荟萃分析.结果 共纳入5篇RCTs、302例肩袖钙化性肌腱炎患者.荟萃分析结果显示:治疗组有效率显著高于对照组[MD=21.15,95%CI(2.84,157.44),P<0.01];治疗组患者视觉模拟评分法(visual analogue scale,VAS)评分显著低于对照组[MD=-3.37,95%CI(-5.25,-1.50),P<0.01];治疗组患者Constant-Murley肩关节功能评分(Constant-Murley score,CMS)明显高于对照组[MD=24.09,95%CI(11.56,36.62),P<0.01];治疗组患者钙沉积平均粒径显著小于对照组[MD=-10.49,95%CI(-19.28,-1.70),P<0.01];治疗组患者钙化灶消失率显著高于对照组[MD=24.24,95%CI(3.35,175.20),P<0.01].结论 体外冲击波治疗肩袖钙化性肌腱炎具有较好的临床效果,可以缓解疼痛,改善肩关节功能,缩减钙沉积平均粒径,提高钙化灶消失率.
