目的 分析基底节区脑出血患者并发医院感染的危险因素,为基底节区脑出血并发医院感染的防治提供参考依据.方法 回顾性分析本院2014年1月至2016年1月收治的391例基底节区脑出血患者的临床资料.计算患者医院感染发生率,采用多因素Logistic回归分析探讨基底节区脑出血并发医院感染的独立危险因素.结果 391例患者中,54例发生医院感染,发生率为13.81%;患者医院感染部位以肺部为主,占53.70%,其次为泌尿系统感染,占22.22%.Logistic多因素回归分析显示,年龄≥65岁、格拉斯哥昏迷评分<8分、白细胞计数≥10×109/L、白蛋白水平<13 g/L、有吸烟史、合并糖尿病、手术治疗及侵入性操作是基底节区脑出血患者并发医院感染的独立危险因素(P<0.05).结论 基底节区脑出血并发医院感染与患者基线资料、生活习惯、疾病史、治疗情况密切相关,据此评估患者医院感染发生风险并予以合理干预有望降低医院感染发生率,改善预后.%Objective To analyze the risk factors of nosocomial infection in patients with cerebral hemorrhage in the basal ganglia region, and to provide reference for the prevention and treatment of cerebral hemorrhage complicated with nosocomial infection.Method The clinical data of 391 cases of basal ganglia hemorrhage patients in our hospital from January 2014 to January 2016 were analyzed retrospectively, the incidence of nosocomial infection were calculated and compared, used multiple Logistic regression summarized the influence of independent risk factors of nosocomial infection in cerebral hemorrhage in basal ganglia.Result Among the 391 patients, 54 patients had nosocomial infection, the incidence rate was 13.81%. The patient's hospital infection was mainly lung, accounted for 53.70%, followed by urinary tract infection, accounted for 22.22%. Logistic regression analysis showed that age≥ 65 years old, Glasgow coma scale< 8 points, white blood cell counts≥ 10×109/L, albumin level< 13 g/L, had smoking history, diabetes, surgery and invasive operation were the independent risk factors of nosocomial infection in cerebral hemorrhage in basal ganglia (P<0.05).Conclusion Cerebral hemorrhage complicated by hospital infection in the basal ganglia region patients with baseline data, living habits, history of disease, treatment are closely related, assessing thenosocomial infection risk and reasonable intervention is expected to reduce the incidence of hospital infection, improve the prognosis.
