Objectives:To analyze and explore the effects of different types of vaginal hysterectomy for uterine leiomyoma and their influence on female sexual function.Methods:180 hysterectomy patients treated in our hospitals were selected and divided into two groups according to the types of surgical resections,with 90 cases in each group.Group A received laparoscopic-assisted vaginal hysterectomy and group B received laparoscopic-assisted subtotal hysterectomy.The effects of different types of hysterectomy and the influence on ovarian function and sexual function were observed in both groups.Results:Operation time,blood loss,postoperative hospital stay,anal exhaust time and related clinical indexes in group B were significantly better than these in group A (P ＜ 0.05).The ovarian function scores and overall sexual function scores of patients under 50 years old in the two groups were notsignificantly different (P ＞ 0.05).The overall sexual function score of group B was significantly higher (P ＜ 0.05).Conclusion:Laparoscopic-assisted subtotal hysterectomy has high reliability and security,which can improve the life quality of patients over 50 years old.%目的:探讨腹腔镜下阴式全子宫切除术和次全切除术两种手术方式治疗子宫肌瘤的临床疗效及对女性性功能的影响.方法:将180例子宫肌瘤患者分为A、B两组,A组90例患者采用腹腔镜下阴式全子宫切除术,B组90例患者采用腹腔镜下阴式次全子宫切除术,对比不同手术方式切除子宫肌瘤的疗效,分析手术方式对A、B两组患者卵巢功能、性功能评分的影响.结果:B组患者在手术时间、失血量、术后住院时间及肛门排气时间相关临床指标均显著优于A组(P＜0.05);B组50岁以下患者较A组在卵巢及性功能评分方面无显著差异(P＞0.05).50岁以上患者相比,B组较A组在卵巢功能评分方面无显著差异(P＞0.05),但B组性功能评分显著高于A组(P＜0.05).结论:腹腔镜辅助阴式次全子宫切除术安全、可靠,疗效较好,有助于提高50岁以上子宫切除患者性生活质量.
