雷公藤治疗寻常型银屑病随机对照试验系统评价

         

摘要

Objective To evaluate the therapeutic effect of Tripterygium Wilfordii on psoriasis vulgaris by systematic review of the randomized controlled trials (RCTs) in treating psoriasis vulgaris. Methods The RCTs of Tripterygium Wilfordii in treating psoriasis vulgaris were retrieved from Cochrane Library (Issue 3, 2012), PubMed (1966-2012), EMbase (1974-2012), Wan Fang Data (1990-2012), VIP (1989-2012), CNKI (1980-2012), Wan Fang Dissertation Data (1990-2012) and Wan Fang Conference Data (1990-2012). Methodological quality assessment and data extraction assessment for methodological quality was using a modification of criterriagive in Cochrane Reviewers' Handbook. RevMan 4.2 was used for Meta-anatysis. Odds ratio (OR) and 95% confidence intervals (95%CI) were used to analyze the efficacy. Results Fourteen papers of RCTs were included. Two papers of Tripterygium Wilfordii plus Retdnoids and 3 papers of Tripterygium Wilfordii plus Diyin Tablet were analyzed by fixed effect model, others were descriptively analyzed. The results showed that Tripterygium Wilfordii was effective on psoriasis vulgaris. Conclusion The therapeutic effect of Tripterygium Wilfordii in treating psoriasis vulgaris is assured, but the evidence of large, multicenter RCTs are urgently needed, so as to provide more convincible evidence.%目的 对雷公藤治疗寻常型银屑病的随机对照临床研究进行系统分析,以期对其疗效作出客观的评价.方法 按纳入标准和排除标准利用计算机检索Cochrane Library(2012年第3期)、PubMed(1966-2012年)、EMbase (1974-2012年)、万方期刊数据库(1990-2012年)、重庆维普中文期刊数据库数据(1989-2012年)、中国知网期刊全文数据库(1980-2012年)、万方学位论文数据库(1990-2012年)和万方会议论文数据库(1990-2012)中有关雷公藤治疗寻常型银屑病的随机对照研究.按照Cochrane系统评价员手册推荐的质量评价标准评价纳入研究质量,用RevMan 4.2软件对同质研究进行Meta分析,用比值比(0R)和95%可信区间(95%CI)作为疗效分析统计量.结果 14个雷公藤治疗寻常型银屑病的随机对照试验纳入本研究,分别对2个雷公藤联合维A酸试验、3个雷公藤联合迪银片试验用固定效应模型进行合并,其他临床试验进行描述性分析.结果显示应用雷公藤治疗寻常型银屑病有效.结论 雷公藤治疗寻常型银屑病有效.但由于纳入研究的方法学质量较低,期待科学设计的临床研究方案,进行严格的多中心、大样本的随机双盲对照试验,以提供高质量的科学文献.
