目的 探讨幽门螺杆菌(Hp)与功能性消化不良(FD)的中医证型及主要症状的关系.方法 共92例符合标准的FD患者参加本次研究,记录患者的Hp情况,对胃脘胀满、胃脘疼痛、上腹部烧灼感、早饱、恶心呕吐、嗳气、疲乏无力、便秘、便溏9个症状进行症状评分.将FD患者分为Hp阳性组和Hp阴性组,观察2组中医证型的分布情况,比较2组消化不良症状的出现率及各症状积分与Hp的关系.结果 Hp阳性患者在湿热壅滞及肝胃郁热证型中分布比例较大,与脾虚气滞、肝胃气滞2个证型间差异有统计学意义.Hp阳性组的症状积分高于Hp阴性组,差异具有统计学意义(P＜0.05).Hp阳性FD患者胃脘疼痛、上腹部烧灼感、嗳气、便秘症状的出现率与Hp阴性患者比较差异有统计学意义(P＜0.05).结论 Hp阳性的FD患者属湿热壅滞及肝胃郁热证较多.Hp阳性患者症状较Hp阴性患者重,以胃脘疼痛、上腹部烧灼感、嗳气、便秘症状出现较多.%Objective To discuss the distribution of Hp in patients with functional dyspepsia (FD) and its relationship with TCM symptoms and syndromes. Methods A total of 92 cases of FD patients was involved in this study and their Hp was observed. The scores of abdominal distension, epigastric pain, epigastric burning, early satiety, fatigue, nausea, eructation, fatigue and constipation were recorded. FD patients were divided into Hp-positive group and Hp-negative group, and their syndromes distribution was observed. The occurrence rate of each symptom of two groups and the relationship between symptom scores and Hp were compared. Results Liver-stomach heat stagnation syndrome and damp-heat accumulation syndrome had a larger proportion in Hp-positive patients, with significant difference compared with the other two syndromes (P<0.05). The total score of symptoms of Hp-positive patients was higher than that of Hp-negative patients with significant difference (P<0.05). The difference in the occurrence of stomachache, upper abdominal burning sensation, eructation and constipation between Hp-negative and Hp-positive was statistically significant (P<0.05). Conclusion More Hp-positive patients attribute to liver-stomach heat stagnation syndrome or damp-heat accumulation syndrome. The symptoms of Hp-positive patients were more severe than Hp-negative patients, and abdominal burning sensation, eructation and constipation were more frequent.
