CT仿真内镜诊断离体猪小肠隆起性病变

         

摘要

Objective To explore the value of CT virtual endoscopy (CTVE) in diagnosis of small intestine protruding lesions. Methods Isolated porcine small intestines was taken as samples and underwent MSCT scanning. The simulated heights of the intestinal wall protuberant lesions were less than 5 mm and 5-10 mm with diameter 2-15 mm. CTVE was made at work station. The detection of simulant protuberant lesions of different sizes and heights were compared. Results With the increase of diameter and height, the sensitivity, specificity, Youden index and crude agreement of CTVE increased. In detection of small intestine simulant protuberant lesions less than 5 mm in height but diameter 11-15 mm,height 5-10 mm with diameter 6-10 mm and diameter 11-15 mm, the sensitivity and specificity of CTVE were all 100%. Conclusion CTVE has fairly high accuracy in detection of small intestine protuberant lesions with diameter and height both more than 5 mm. Compared with the diameter, the height of lesions has greater influence on the accuracy.%目的 探讨CT仿真内镜(CTVE)技术诊断小肠隆起性病变的价值.方法 取离体猪小肠标本,于内壁模拟高度分别为<5 mm及5~10 mm、直径2~15 mm、形态各异的隆起性病变,并行MSCT扫描.利用工作站进行CTVE成像,比较不同大小、高度的模拟隆起性病变的检出情况.结果 随着模拟病变直径、高度的增加,CTVE技术的敏感度、特异度、约登指数和粗一致性均升高.对于高度<5 mm而直径11~15 mm、高度为5~10 mm而直径为6~10 mm以及直径11~15 mm的模拟隆起性病变,CTVE的敏感度和特异度均为100%.结论 CTVE对于直径和高度均>5 mm的小肠隆起性病变检出的准确率较高;与病变直径相比,病变高度对CT仿真内镜检出的准确度影响更大.
