目的探讨螺旋CT对小肠肿瘤的诊断及术前可切除性估计的价值。方法前瞻性对26例疑有小肠肿rn瘤的患者行螺旋CT检查，检查前肌注盐酸654-210mg以减少肠管蠕动，口服足量温开水充盈肠管；先平扫，然后经rn肘静脉以3ml/s注射Omnipaque(300mgI/ml)90ml行动态增强扫描。结果 26例均获病理证实，其中腺癌21例rn(80.7％)，平滑肌肉瘤3例，类癌1例，淋巴瘤1例。螺旋CT对小肠肿瘤检出的敏感性为100％，准确性为96.2％，rn对可切除性和不可切除性估计的准确性分别为87.5％和100％。结论螺旋CT对小肠肿瘤特别是十二指肠肿瘤的rn检出率高；是估计可切除性最可靠的方法。%Purpose To investigate the value of helical CT for diagnosing small bowel neoplasm and predicting tumor rnresectability. Methods Helical CT scans were prospectively performed in 26 patients before and after infusion of conrntrast medium (Omnipaque, 300mgI/ml) 90ml at rate 3ml/s. Before CT examination, all patients were injected 10mgrn654-2 to minimize bowel movement, then took warm boiled water as much as possible to full the bowel. Results Allrn26 cases were confirmed with pathology including 21 adenocarcinoma (80.7%), three leiomyosarcoma, one carcinoid rntumor,one lymphoma. The detectability of tumor was 100%, accuracy was 96.2%. The accuracy of predicting resectrnability and unresectability was 87.5% and 100% respectively. Conclusion Helical CT has high value for detecting rnsmall bowel neoplasm and was the best method for predicting tumor resectability.
