应用磁性附着体修复可摘局部义齿受损基牙的临床研究

         

摘要

目的:探讨应用磁性附着体修复可摘局部义齿受损基牙的可行性.方法:选择可摘局部义齿功能完好,只因固位基牙受损(未至拔除条件)而无法正常戴用的患者63例,受损基牙74颗,做相应基础治疗后,对受损基牙行磁性附着体覆盖义齿修复,比较修复前后义齿固位力、咀嚼功能、牙周健康状况和美观方面的差异.结果:磁性附着体修复后,义齿固位力、稳定性、咀嚼效率、美观等方面均能达到基牙受损前水平.义齿戴入后适应快,无明显不适感.磁性附着体修复74颗受损基牙,初次试戴后1周内疗效满意率100%.3个月后复查73枚,疗效好72枚;1颗基牙发生牙龈炎.1年后复查67枚,疗效好58枚;8颗基牙发生牙龈炎;牙周病加重至基牙拔除1颗.结论:利用磁性附着体修复因固位基牙受损而无法正常戴用的可摘局部义齿,能较大限度的保留、利用基牙,延长义齿的使用寿命,减少患者的经济负担和就诊次数.义齿在固位、稳定、咀嚼效率、摘戴方便等方面性能明显提高.%Objective: To explore the feasibility of applying magnetic attachment in repairing traditional removable partial denture that could not work normally due to damages of the retentive abutment teeth. Methods: Patients between 2008 and 2011 were selected in this study, whose removable partial dentures were in good condition, but could not be worn normally due to damages of their major abutment teeth(not met the criteria for pulling off). 63 cases with 74 damaged abutment teeth were involved. After relevant basic treatment, the damaged abutment teeth were restored through covering the denture with magnetic attachment. Dentures before and after the restoration were compared in the aspects of their retentive force, chewing functions, and appearances. Results: After the magnetic attachment was performed, the denture's retention force, stability, chewing ability and appearance either met or exceeded the standards before magnetic attachment utilization. The denture showed a high adaptability and caused no obvious discomfort after it was inserted. 74 damaged abutment teeth were restored with magnetic attachment. After one week of their first trial, 100% satisfaction was achieved. Three months later, 73 teeth were followed up. 72 of them showed good treatment effects, which was 98.23% of the total number. One abutment tooth, which was 1.37% of the total number, had gingivitis, and the treatment effect was average. One year later, 67 of them were followed up. 58 of them showed good treatment effects, which was 86.56% of the total number. 8 abutment teeth, which were 11.94% of the total, had gingivitis, and treatment effects were average. One abutment tooth had to be pulled out due to the worsening periodontal disease; the treatment effect was poor, and constitutes 1.37% of the total. Conclusion: Using magnetic attachment to repair removable partial dentures that cannot be worn normally due to damages to abutment tooth would promote retention and make use of abutment teeth to the maximum extent, extend dentures'duration, reduce pa-tients' financial burden and the number of clinical visits. The dentures were improved obviously in the aspects of retention, stability, chewing ability and wearing convenience. It also improved dentures'appearance and comfortableness, and reduced the dentures' adaptability time. This method is simple, easy, effective and, therefore, suitable for broader clinical application.
