Objective To investigate the role of endometrial thickness,endometrial pattern,parameters of uterine artery, endometrial and subendometrial blood flows measured by transvaginal color Doppler ultrasound in the prediction of endometrial receptivity during in vitro fertilization-embryo transfer(IVF-ET )cycles.Methods One hundred and twenty patients undergoing IVF cycles were recruited in this study.Transvaginal color Doppler ultrasound was performed to determine endometrial thickness, endometrial pattern, pulsatility index (PI), resisitance index ( RI), systolic/diastolic ratio(S/D) of uterine artery and endometrial and subendometrial blood flows by experienced sonographers on the day of human chorionic gonadotropin(HCG) injection.Statistical comparison was carried out by Mann-Whitney test and chi-square test.Statistic analysis were performed using SPSS 13.0.The two-tailed value of P＜0.05 was considered statistically significant.Results There was no significant difference (P＞0.05) between the pregnant group and non-pregnant group in endometrial thickness, PI、 RI、 S/D of uterine artery.Patients in the pregnant group had lower PI,RI and S/D of endometrial and subendometrial blood flows than those in the non-pregnant group (P＜0.05),and statistical comparison had significant difference.The pregnance rate was significantly higher in the group with triplelined endometrium than that in the group with non-triplelined endometrium.Conclusions Endometrial pattern,endometrial and subendometria blood flows measured by transvaginal color Doppler ultrasound can be as a reliable indicator to assess endometrial receptivity and to predict the pregnancy outcome.%目的 探讨经阴道彩色多普勒超声检测子宫内膜厚度、类型、子宫动脉血流动力学参数、内膜及内膜下血流对体外受精-胚胎移植(IVF-ET)中子宫内膜容受性的预测价值.方法 应用经阴道彩色多普勒超声,对120例接受IVF-ET的不孕症患者于控制性超排卵(COH)周期人绒毛膜促性腺激素(HCG)注射日进行子宫内膜厚度、类型、子宫动脉血流动力学参数、内膜及内膜下血流检测,根据妊娠结果分为妊娠组与未妊娠组,比较两组之间各参数的差异.结果 120例患者中有3例未检测到内膜血流,2例未检测到内膜及内膜下血流,其余115例内膜和内膜下血流均显示.120例患者临床妊娠39例,妊娠率为32.5%,妊娠组与未妊娠组间子宫内膜厚度、子宫动脉PI、RI、S/D比较,差异无显著性意义(P>0.05);内膜回声为三线型者妊娠率高于非三线型者,两组间比较差异均有显著性意义(P<0.05);妊娠组内膜及内膜下血流PI、RI、S/D均明显低于非妊娠组,两组间比较差异均具有显著性意义(P<0.05).结论 经阴道彩色多普勒超声检测子宫内膜类型、内膜及内膜下血流,可用于评价子宫内膜容受性、预测IVF-ET结局.
