Objective To explore the influence of combined treatment of insulin aspart and huangqi sijunzi decoction on pregnancy outcomes of pregnant women with gestational diabetes.Methods Ninety patients with gestational diabetes admitted in obstetrics department of Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 2014 to June 2016 were selected and divided into control group and observation group through random number table, with 45 cases in each group.Patients in the control group were given insulin aspart treatment, while patients in the observation group were given huangqi sijunzi decoction on basis of insulin aspart therapy.Clinical curative effect, blood sugar change and pregnancy outcomes were compared between two groups.Results Total effective rate in the observation group was significantly higher than that in the control group (χ2=6.75, P<0.05).Average dose of insulin aspart used in the observation group was significantly lower than that in the control group (χ2=4.63, P<0.05).Fasting glucose, 2h postprandial glucose and glycosylated hemoglobin levels after treatment in both groups were significantly lowered compared with those before treatment (t value was 3.69, 3.87, 3.93, 5.89, 6.03 and 6.29 respectively, all P<0.05).There was no significant difference in fasting glucose, 2h postprandial glucose and glycosylated hemoglobin levels after treatment between two groups (t value was 1.75, 1.03 and 1.98, respectively, all P>0.05).Incidences of neonatal asphyxia, fetal distress, neonatal hypoglycemia, abnormal neonatal weight, gestational hypertension, polyhydramnios, preterm birth and late abortion in the observation group were significantly lower than those in the control group (χ2 value was 6.25, 5.78, 6.10, 5.99, 6.35, 6.14, 5.88 and 6.28, respectively, all P<0.05).Conclusion Insulin aspart combined with huangqi sijunzi decoction has good effect in treatment of patients with gestational diabetes mellitus.Keeping patient''s glucose level in good status, it can reduce dose of insulin and provide good protection for maternal and infant outcomes.%目的 分析诺和锐特充联合黄芪四君子汤治疗对妊娠糖尿病患者妊娠结局的影响.方法 选取山东省中医药大学附属医院产科自2014年6月至2016年6月收治的90例妊娠糖尿病患者,采取随机数字表法分为对照组与观察组,每组各45例,对照组给予诺和锐特充治疗,观察在对照组基础上加用黄芪四君子汤治疗,对比两组临床疗效、血糖变化以及妊娠结局.结果 观察组总有效率显著高于对照组(χ2=6.75,P<0.05),且诺和锐特充平均使用剂量显著低于对照组(t=4.63,P<0.05).两组患者治疗后空腹血糖、餐后2h血糖以及糖化血红蛋白水平均较治疗前显著降低(t值分别为3.69、3.87、3.93、5.89、6.03、6.29,均P<0.05),两组患者治疗后空腹血糖、餐后2h血糖及糖化血红蛋白水平比较均无显著性差异(t值分别为1.75、1.03、1.98,均P>0.05).观察组新生儿窒息、胎儿窘迫、新生儿低血糖、新生儿体重、妊娠期高血压疾病、羊水过多、早产、晚期流产发生率均较对照组显著降低(χ2值分别为6.25、5.78、6.10、5.99、6.35、6.14、5.88、6.28,均P<0.05).结论 诺和锐特充联合黄芪四君子汤治疗妊娠糖尿病患者的疗效显著,能在保障患者良好血糖水平的同时,明显减少胰岛素的使用剂量,为母婴妊娠结局提高良好保障.
