Objective To study the correlation between delivery mode and childbirth outcomes of scarred uterus pregnancy .Methods The data of 105 pregnant women with scarred uterus admitted in Third People ’ s Hospital of Zhangjiagang City during the period of January 2011 to May 2014 were retrospectively analyzed .The cases were divided into vaginal delivery group and cesarean section group according to the mode of delivery , and the childbirth outcomes and complications of two groups were compared .Results The amount of bleeding during operation, postpartum bleeding and hospitalization length of the vaginal delivery group were significantly decreased than those of the cesarean section group (t value was 21.893, 31.130 and 8.542, respectively, all P<0.01).The incidence of infection and postpartum puerperal fever in the vaginal delivery group was significantly lower than that in the cesarean section group (χ2 value was 3.889 and 4.024, respectively, both P<0.05).Neonatal weight of the vaginal delivery group was significantly decreased than that in the cesarean section group (t=3.839,P<0.01), but the Apgar score was remarkably higher than that in the cesarean section group (t=4.674,P<0.01). There was no significant difference in the incidence of neonatal asphyxia and neonatal infection rate between two groups (χ2 value was 2.538 and 0.041, respectively, both P>0.05).Conclusion It is good both for mother and infant to conduct vaginal delivery when there are vaginal delivery indications , and it has important clinical reference value .%目的：探讨瘢痕子宫再次妊娠的分娩方式与分娩结局的相关性。方法回顾性分析2011年1月至2014年5月张家港市第三人民医院收治的105例瘢痕子宫再次妊娠分娩孕妇，按照分娩方式将其分为阴道分娩组和剖宫产组，比较两组的分娩结局及并发症情况。结果阴道分娩组的术中出血量、产后出血量、住院时间均少于剖宫产组，差异有统计学意义（t值分别为21．893、31．130、8．542，均P＜0．01），阴道分娩组产褥感染发生率、产后发热率均低于剖宫产组，差异有统计学意义（χ2值分别为3．889、4．024，均P＜0．05）。阴道分娩组新生儿体重显著低于剖宫产组（t＝3．839，P＜0．01），Apgar评分显著高于剖宫产组（t＝4．674，P＜0．01）。两组新生儿窒息率和新生儿感染率比较，差异无统计学意义（χ2值分别为2．538、0．041，均P＞0．05）。结论在具备阴道分娩指征的条件下进行阴道试产对母婴结局均有利，临床有重要参考价值。
