目的 研究三磷酸胞苷二钠治疗新生儿缺氧缺血性脑病的临床疗效.方法 将80例新生儿缺氧缺血性脑病患儿随机分成治疗组和对照组.对照组40例给予常规治疗,治疗组40例在常规治疗的基础上给予三磷酸胞苷二钠20mg/d,静脉滴注,每天1次,观察两组临床表现和实施贝利婴幼儿发展量表的测评.结果 治疗组的意识障碍、惊厥、前囟张力、肌张力、原始反射、呼吸节律较对照组均明显缩短,经比较差异有统计学意义(t值分别为2.58、2.42、2.61、3.29、2.87、2.36,均P<0.05);贝利婴幼儿发展量表测试结果显示:在3、6和9月龄时,治疗组智力发展指数粗分和标准分与对照组比较差异均有统计学意义(t分别为2.72、2.81;2.34、2.53;2.17、2.49,均P<0.05),运动发展指数粗分和标准分与对照组比较差异均有统计学意义(t分别为2.42、2.58;2.34、2.42;2.11、2.29,均P<0.05).结论 三磷酸胞苷二钠能有效地减轻新生儿缺氧缺血性脑病的临床症状,缩短病程,减轻后遗症.%Objective To observe theraprutic effect of disodium cytidine triphosphate for hypoxic-ischemic encephalopathy (HIE) of neonate. Methods 80 neonates with HIE were randomly divided into treatment group (40 cases) and control group (40 cases ). The neonates in the control group were treated with routine methods for consecutive 10 days, while those in the treatment group were treated with disodium cytidine triphosphate (20mg/d, intrvenous drop once per day for consecutive 10 days) on basis of the routine treatment. The clinical manifestations of the neonates were recorded, and the neonates were assessed with Bayley Scales of Infant Development (BSID).Results The recovery times of clinical symptoms and physical signs such as disturbance of conciousness, convulsion, bulging of anterior fontanelle, high muscular tension, primitive refluxes and respiratory rhythm of the neonates in the treatment group were shorter significantly than those of the neonates in the control group ( t = 2.58, 2.42, 2.61, 3.29, 2.87, 2.36 respectively, all P ＜ 0.05 ). BSID results showned that there were significant differences between the two groups in MDI rough score and standard score of the neonates at 3, 6 and 9 months of age ( t = 2.72, 2.81; 2.34, 2.53; 2.17, 2.49 respectively, all P ＜ 0.05 ), and also there were significant differences between the two groups in PDI rough score and standard score ( t = 2.42, 2.58; 2.34, 2.42; 2.11,2.29 respectively, all P ＜ 0. 05 ). Conclusion Disodium cytidine triphosphate can effectively alleviate clinical symptoms, shorten the course of disease and ease sequela of the neonates with HIE.
