目的 评价血清肌酸激酶对预测输卵管妊娠破裂的临床价值.方法 选择输卵管妊娠破裂型55例为实验组,输卵管妊娠流产型45例为对照组1,正常宫内妊娠50例为对照组2,取静脉血测定血清β-人绒毛膜促性腺激素和肌酸激酶,并进行统计分析.结果 输卵管妊娠破裂型血清肌酸激酶水平明显高于输卵管妊娠流产型,两者比较有显著性差异(t=4.876,P<0.01);血清肌酸激酶以65U/L为临界值预测输卵管妊娠破裂的敏感性、特异性、阳性预测值、阴性预测值、符合率分别为93.8%、66.7%、95.8%、57.1%、90.9%.输卵管妊娠流产型血清肌酸激酶水平略高于正常宫内妊娠,两者比较无显著性差异(t=2.224,P>0.05).结论 血清肌酸激酶可作为诊断输卵管妊娠破裂的生化指标.%Objective To evaluate diagnostic value of serum creatine kinase (SCK) for predicting rupture of tubal pregnancy. Methods 55 pregnant women with ruptured tubal pregnancy were selected in experimental group, 45 pregnant women with tubal pregnancy miscarriage were selected in control group Ⅰ and 50 normal women with intrauterine pregnancy were selected in control group Ⅱ. Venous blood samples of all pregnant women were taken to detect serum levels of β-hCG and SCK. The data were analyzed statistically. Results SCK level of women with reptured tubal pregnancy was significantly higher than that of women with tubal pregnancy miscarriage, there was a significant difference ( t = 4.876, P ＜ 0.01 ). Sensitivity ( Se), specificity ( Sp), positive predictive value ( PPV), negative predictive value ( NPV ) and consistent rate(CR) regarding 65U/L of SCK as the cutoff value to predict rupture of tubal pregnancy were 93.8％, 66.7％, 95.8％,57.1％, 90. 9％ respectively. SCK level of women with tubal pregnancy miscarriage was slightly higher than women with normal intrauterine pregnancy, but there was no significant difference between the two groups ( t = 2.224, P ＞ 0.05 ). Conclusion SCK could be regarded as a biochemical marker to diagnose rupture of tubal pregnancy.
