Objective To investage prevalences of cervical cancer and cervical intraepithelial neoplasia(CIN) among women in villages or towns in Miyun county, Beijing. Methods The screening data of cervical cancer of inhabitant women aged 25-65 years in 4 villages and 5 communities of downtowns in Miyun county were retrospectively analyzed. Results Totally, 20236 women were screened, including 14 433 women in villages and 5 923 women in towns. The positive rate of cervical cytologyical examination were 2. 49％ (507/20 356 ). 219 women received HPV-DNA detection, among them, the high-rsk HPV (HR-HPV) infection rate in village women group was 69.37％ (77/111 ), and that in town women group was 67.59％ (73/108), there was no significant difference between the two women groups(x2 ＝0. 080, P ＞ 0.05 ). The pathological results of colpoecopy and cervical biopsy showed that in village women group, positive rate of CIN Ⅱ was 36.66％ (33/90), positive rate of CIN Ⅲ was 50.00％ (45/90), and positive rate of cervical cancer was 3.33％ (3/90), while in town women group, positive rate of CIN Ⅱ was 45.00％ ( 36/80), positive rate of CIN Ⅲ was 45.00％ (36/80) and positive rate of cervical cancer was 2.50％ ( 2/80 ), there was no significant difference between the two women groups(x2 = 1. 220, 0. 424, 0. 103 respectively, all P ＞ 0. 05 ). Conclusion In Miyun county, there are no significant differences between village women group and downtown women group in prevalences of cervical cancer and CIN.%目的 了解本地区农村和城镇妇女宫颈癌及癌前病变的患病情况.方法 回顾性分析2009年8月～2010年7月间,密云县妇幼保健院对县域内4个农村乡镇、5个社区的25～65岁户籍妇女进行宫颈癌普查的临床资料.结果 普查总数为20 356例,其中农村妇女14 433例、城镇妇女5 923例,细胞学检查阳性率为2.49%(507/20 356);进行HPV-DNA检测219例,农村组HR-HPV感染率为69.37%(77/111),城镇组HR-HPV感染率为67.59%(73/108),两组比较差异无统计学意义(χ2=0.080,P>0.05).病理检查结果,农村组宫颈上皮内瘤变Ⅱ阳性率36.67%(33/90),宫颈上皮内瘤变Ⅲ阳性率为50.00%(45/90),宫颈癌阳性率3.33%(3/90);城镇组宫颈上皮内瘤变Ⅱ阳性率45.00%(36/80),宫颈上皮内瘤变Ⅲ阳性率为45.00%(36/80),宫颈癌阳性率2.50%(2/80),经比较差异无统计学意义(χ2分别为1.220、0.424、0.103,P>0.05).结论 本地区农村和城镇妇女宫颈癌及癌前病变患病率无明显差异.
