Objective:To probe into the causes of dispensing errors in outpatient pharmacy,and analyze the effects of quality control circle activities on reducing dispensing errors in outpatient pharmacy.METHODS: 588 586 prescriptions extracted from Zhuhai Hospitals of Traditional Chinese and Western Medicine in 2016 were analyzed by retrospective analysis and tree analysis method,the causes were investigated and effects of quality control circle activities were evaluated.RESULTS: Of the 648 cases of dispensing errors,603 cases were internal dispensing errors,there was no effects on patients after timely adjustment by clinical pharmacists;however,45 cases had few effects on patients and nurses,with the rate of 0.765%目的:探讨门诊药房处方调剂差错的原因,分析门诊药房开展品管圈活动对降低处方调剂差错的影响.方法:采用回顾性分析法和树状分析法对珠海市中西医结合医院2016年门诊西药房588 586张处方进行分析,探讨其原因并评价开展品管圈活动的效果.结果:648张调剂差错处方中,603张处方为内部调剂差错,临床药师及时更正后未对患者造成影响,45张处方对患者或护士工作造成了一定影响,出门差错率约为0.765(‰)(45/588 586);品管圈活动后,门诊处方调剂差错率由16.795(‰)(508/302 468)降至4.893(‰)(140/286 118).结论:开展品管圈活动可提高药学部核心竞争力,降低调剂差错率,持续改进医疗质量,确保医疗安全.
