目的:了解2014—2016年首都医科大学附属北京安贞医院(以下简称"我院")胰岛素制剂及口服降糖药使用情况,评价我院抗糖尿病药应用状况及其合理性,为临床用药提供指导意见.方法:利用我院计算机管理软件检索2014—2016年抗糖尿病药的使用数据,以世界卫生组织推荐的限定日剂量(defined daily dose,DDD)为主要参考标准,分析我院抗糖尿病药的应用情况.结果:3年来,我院使用的抗糖尿病药共涉及6大类28个品规,其销售金额呈上升趋势,从2014年的5205.08万元升至2016年的6164.86万元,总增幅达18.44％;胰岛素制剂的销售金额为5289.59万元,占抗糖尿病药销售金额的30.38％,其中普通胰岛素注射液(万邦)的用药频度排序居首位;口服降糖药的使用仍然以α-葡萄糖苷酶抑制剂、磺酰脲类药物和双胍类药物为主,其中阿卡波糖片(拜糖苹)、盐酸二甲双胍片(格华止)和瑞格列奈片(诺和龙)的销售金额排序、用药频度排序均居前3位.结论:我院抗糖尿病药品种齐全,使用基本合理.使用的胰岛素制剂以速效胰岛素为主,口服降糖药中α-葡萄糖苷酶抑制剂为临床一线用药,胰岛素增敏剂的使用较少.%OBJECTIVE:To investigate the the application of insulin preparation and oral antidiabetics in Beijing Anzhen Hospital Affiliated to Capital Medical University ( hereinafter referred to as "our hospital") , and to evaluate the application status and its rationality , so as to provide guidance for clinical medication .METHODS: Data of the application of antidiabetics in our hospital during 2014-2016 was retrieved from hospital management software , and it was analyzed according to the reference standards of defined daily dose ( DDD) recommended by the WHO .RESULTS:In the recent 3 years, 28 kinds of 6 categories of antidiabetics were applied in our hospital , the consumption sum had presented a rising trend from 52.0508 million yuan in 2014 to 61.6486 million yuan in 2016, with a total growing rate of 18.44％;the consumption sum of insulin preparation was 52.8959 million, accounted for 30.38％ of that of the antidiabetics , the general insulin injection ( Wanbang ) ranked the first in DDDs; the application of oral antidiabetics mainly were α-glucosidase inhibitors , sulfonylureas and biguanides , of which the acarbose ( Baitangping ) , metformin hydrochloride ( Gehuazhi ) and repaglinide ( Nuohelong ) ranked top 3 in both of consumption sum and DDDs . CONCLUSIONS:The application of antidiabetics in our hospital is basically rational in a complete range of categories . Rapid-acting insulin is the mainly used insulin preparation , the application of α-glucosidase inhibitors of oral antidiabetics is in the front line of clinic , and insulin sensitizer is in a less application in our hospital .
