目的：了解航天中心医院（以下简称“我院”）住院患者医嘱不合理用药情况，促进临床合理用药。方法：采用回顾性分析方法，统计我院2014年1月—2015年12月27573例住院患者医嘱用药情况，对不合理用药进行分析。结果：共341例住院患者的医嘱用药不合理，涉及170种药品，其中抗感染药为最多（36种，占21.18％），其次分别为消化系统用药，营养药、酶制剂和调节水、电解质及酸碱平衡药，心血管系统用药等；不合理用药类型主要为药品用法不适宜（105例，占30.79％），其次分别为存在配伍禁忌或不良相互作用、剂量不适宜、重复用药等。结论：临床药师应进一步加强对住院医嘱的审查，及时发现不合理用药并及时反馈给临床医师，确保患者的用药安全。%OBJECTIVE:To investigate the irrational drug use in inpatient medical orders in Aerospace Center Hospital( hereinafter referred to as “our hospital”) , so as to promote the rational drug use in clinic .METHODS:By retrospective analysis method , 27 573 inpatient medical orders from Jan .2014 to Dec.2015 were statistically collected, and the irrational drug use was analyzed .RESULTS: Totally 341 inpatient medical orders were irrational , 170 drugs were involved , among which anti-infectious drugs took the lead ( 36 , 21.18%) , followed by digestive system drugs, nutriceutical drugs, enzymic preparations and regulation water , electrolyte and acid-base balance drugs, cardiovascular system drugs , etc..The types of irrational application were mainly improper usage ( 105 cases, 30.79%) , followed by incompatibility , adverse drug interactions , improper dosage and repeated medication , etc.. CONCLUSIONS:The clinical pharmacists should further strengthen the review of inpatient prescriptions so as to find out the irrational phenomenon and give timely feedback to the clinicians , and ensure the medication safety for patients .
