OBJECTIVE:To explore the pattern of pharmaceutical care that fit for the community .METHODS:A questionnaire was designed to investigate the situation of medication safety knowledge in residents and the drug treatment of chronic disease in Zhuhai Hengqin community .The medication data were extracted from the public health management system and the HIS of Zhuhai People's Hospital Hengqin Outpatient Department , which were statistically analyzed by using Excel software .RESULTS: The attitudes of interviewed residents from Zhuhai Hengqin community toward the participation of clinical pharmacists in pharmaceutical care was significantly positive , most of respondents were lack of medication safety knowledge and had a relatively conservative concept , yet network technique could be accepted in a certain extent .Chronic disease were generally lack of management and patients didn't attach enough importance to their own diseases , which was still plenty of room for improvement .CONCLUSIONS:There are hidden perils in rational drug use about common and chronic disease in residents who have obvious demand for community pharmaceutical care .According to the current problems , it is necessary to establish the pattern of pharmaceutical care that including public education , medicine treatment management , family follow-up, medication review and utilization analysis and effects evaluation .%目的：探讨适合社区的药学服务模式。方法：设计问卷，深入调查珠海市横琴新区社区居民安全用药常识及慢性病患者药物治疗情况，从公共卫生管理系统和珠海市人民医院横琴门诊部的医院信息系统中获取慢性病患者的用药数据，采用Excel软件对相关数据资料进行统计分析。结果：受访的珠海市横琴新区居民对药师参与社区药学服务的态度积极，大部分受访者对安全用药常识的了解较片面，观念相对保守，但对网络技术有一定的接受度；慢性病患者的用药普遍缺乏管理，其对自身疾病的认识和重视程度不够，有很大的改进空间。结论：居民存在常见病和慢性病的合理用药隐患，对药学服务有明显需求。根据现存问题，应建立以公共教育、药物治疗管理、家庭随访、家庭用药评价和利用度分析、效果评价等5项内容为核心的社区药学服务模式。
