目的：探讨干预前后天津市儿童医院（以下简称“我院”）抗菌药物临床应用的合理性，评估干预效果，促进合理用药。方法：提取我院2011年4月（干预前）、2012年4月（干预后）以及2013—2015年（持续质量改进期间）门、急诊及住院患者使用抗菌药物的相关数据，对比干预前后各项指标的变化情况。结果：干预前、干预后及2015年（持续质量改进第3年），门诊患者抗菌药物使用率分别为38．45％、30．70％、23．62％，急诊患者抗菌药物使用率分别为59．45％、50．50％、35．99％，我院住院患者抗菌药物使用率分别为87．39％、79．20％、62．05％，Ⅰ类切口手术围术期抗菌药物预防性使用率分别为39．22％、17．80％、10．01％，治疗性用药微生物送检率分别为56．85％、67．30％、74．69％；与干预前比较，干预后和2015年（持续质量改进第3年）各项指标均明显改善，差异均有统计学意义（ P＜0．05）。结论：开展抗菌药物临床应用专项整治活动，实施干预措施后，我院抗菌药物临床应用的各项指标都有了明显改善，合理用药水平明显提高。%OBJECTIVE:To probe into the clinical application rationality of antibiotics in Tianjin Children’s Hospital( hereinafter referred to as “our hospital”) before and after intervention, so as to promote the rational application of drugs.METHODS: The related data of application of antibiotics for patients of outpatient, emergency and inpatient department were extracted from our hospital in Apr.2011 ( before intervention ) , Apr.2012 ( after intervention) , and from Jan.2013 to Dec.2015 ( the continuous quality improvement) , changes of various indexes were compared before and after intervention.RESULTS: The application rate of antibiotics in inpatient department before and after intervention and 2015 ( the 3rd year of continuous quality improvement) were respectively 87.39%, 79.20%and 62.05%;in outpatient department department were respectively 38.45%, 30.70%and 23.62%; and in emergency department were respectively 59.45%, 50.50% and 35.99%.The preventive application rate of antibiotics in type I incision were respectively 39.22%, 17.80% and 10.01%; the pathogenic detection rate were respectively 56.85%, 67.30% and 74.69%.Compared with before intervention, various indexes after intervention and 2015 ( the 3rd year of continuous quality improvement ) had been significantly improved, with statistically significant difference ( P <0.05 ) .CONCLUSIONS: Through the development of special rectification and the the implementation of intervention, the related data of antibiotics had been significantly improved, in addition of the level of rational application of antibiotics.
