OBJECTIVE:To systematically review the efficacy and safety of modified Sini decoction combined with chemical drugs in treatment of chronic heart failure .METHODS: CBM, CNKI, VIP, Wanfang data base were randomly retrieved to collect the randomized controlled trials ( RCTs ) of the modified Sini decoction combined with chemical drugs in treatment of chronic heart failure till Aug .9th, 2016.After two reviewers separately screened literature according to the inclusion and exclusion criteria , extracted data and assessed the methodological quality of the included studies , data were analyzed with RevMan 5.3 software .RESULTS:Totally 9 RCTs were included with 693 patients involved , among which 348 cases for treatment group and 345 cases for control group .According to the Meta-analysis , modified Sini decoction combined with conventional chemical drugs treatment were more efficiently in improving clinical total effective rate ( RR =1.27,95%CI =1.16-1.39,P <0.000 01), increasing left ventricular ejection fraction(LVEF) (WMD =5.29,95%CI =3.21-7.36,P <0.000 01), and decreasing traditional Chinese medicine symptom score(WMD=-7.52,95%CI=-13.61--1.44,P=0.02),lifting stroke volume(SV)(WMD=6.43,95%CI=2.61-10.26,P=0.001 0), compared with conventional chemical drugs treatment alone .CONCLUSIONS:Conventional chemical drugs treatment combined with modified Sini decoction in treatment of chronic heart failure can improve the clinical efficacy .%目的：系统评价加味四逆汤联合化学药治疗慢性心力衰竭的临床疗效及安全性。方法：计算机检索中国期刊全文数据库、中国生物医学数据库、中文科技期刊全文数据库、万方数据在线知识服务平台，全面收集有关加味四逆汤联合化学药治疗慢性心力衰竭的随机对照试验，检索时限均从建库至2016年8月9日。由2位评价员按照纳入与排除标准独立筛选文献、提取资料和评价纳入研究的方法学质量后，采用Rev Man 5.3软件进行文献荟萃（Meta）分析。结果：最终纳入9个随机对照试验，共693例慢性心力衰竭患者，其中治疗组348例，对照组345例。 Meta分析结果显示，在化学药常规治疗的基础上加用加味四逆汤能显著提高慢性心力衰竭患者临床总有效率（RR＝1.27，95％CI＝1.16～1.39，P＜0.00001]，显著增加左心室射血分数（WMD＝5.29，95％CI＝3.21～7.36，P＜0.00001），显著降低中医症状评分（WMD＝－7.52，95％CI＝－13.61～－1.44，P＝0.02），并显著增加每搏输出量（WMD＝6.43，95％CI＝2.61～10.26，P＝0.0010）。结论：在化学药基础上加用加味四逆汤治疗心力衰竭，可提高临床疗效。
