OBJECTIVE:To explore the role of clinic pharmacist in warfarin anticoagulation treatment . METHODS: Clinic pharmacist participated in consultation and provided pharmaceutical care for one patient with cerebral embolism following aortic mechanical valve replacement.RESULTS:Clinical pharmacists analyzed the causes of cerebral embolism, adjusted warfarin anticoagulation therapy and provided pharmaceutical care for the patient. CONCLUSIONS:Clinical pharmacist's involvement in warfarin anticoagulant treatment may lead to increase of success rate yet decease of the risk of anticoagulant therapy.%目的：探讨临床药师在华法林抗凝治疗中的作用。方法：临床药师参与会诊1例接受主动脉瓣机械瓣膜置换术后出现脑栓塞的案例，提供药学服务。结果：临床药师分析患者脑栓塞发生的原因，调整华法林抗凝方案，并为患者实施药学监护。结论：临床药师参与华法林抗凝治疗，可提高抗凝治疗成功率，降低患者抗凝风险。
