目的：了解医院门、急诊处方的药物使用情况，以减少药品不良反应发生率，促进合理用药。方法：随机抽取2013年每月门、急诊处方400张，共计4800张，根据《医院处方点评管理规范（试行）》进行点评、分析。结果：4800张处方中，不合理处方248张；不合理用药主要表现为用药与临床诊断不相符、用法与用量不适宜、联合用药不适宜、有配伍禁忌、溶剂使用不当等。结论：应定期进行处方点评，分析临床用药存在的问题，及时发现、合理干预，以提高临床合理用药水平。%OBJECTIVE:To investigate the drug use in the outpatient and emergency department so as to reduce the incidence of adverse drug reactions and promote rational use of drugs. METHODS: 400 outpatient & emergency prescriptions collected monthly ( 4 800 prescriptions in total ) were reviewed in accordance with to the “Management Practices on Hospital Prescription Review( Trial)”. RESULTS:248 outpatient prescriptions were unqualified, and the irrational use of drugs manifested as incompatibility between drug use and the clinical diagnosis, inappropriate in usage& dosage or drug combination, incompatibility and improper use of solvents etc. CONCLUSIONS: Regular review on prescriptions and analysis of the problems in clinical drug use and timely intervention can help improve clinicians�level in rational use of drugs.
