目的：探讨复旦大学附属眼耳鼻喉科医院（以下简称“我院”）静脉配置中心医嘱不合理情况，为临床合理用药提供参考。方法：回顾性分析我院静脉药物配置中心2013年6月—2014年5月静脉用药审核医嘱中发现的528份不合理医嘱。结果：不合理医嘱主要为给药频次、浓度不当，溶剂选用和溶剂量不当，给药时间、途径不当以及配伍禁忌等。结论：药师应运用药学专业知识审核医嘱，及时纠正不合理医嘱，提高临床用药水平。%OBJECTIVE:To analyze the irrational prescriptions in pharmacy intravenous admixture services ( PIVAS ) of Eye&Ent Hospital of Fudan University ( hereinafter referred to as “our hospital”) for reference of rational use of drugs. METHODS:We retrospectively analyzed a total of 528 irrational prescriptions detected from May 2013 to April 2014 in PIVAS of our hospital. RESULTS: The irrationality of the irrational prescriptions manifested as improper in medication frequency, drug concentration, choice and dosage of solvents, timing and route of administration and incompatibility etc. CONCLUSIONS:Pharmacist�s review of prescriptions by making full use of professional knowledge on pharmacy contributes to timely correction of the irrational prescriptions and improvement of the level of clinical medication.
