OBJECTIVE: To investigate the current status and existing problems of outpatient prescriptions of traditional Chinese medicine (TCM) decoction pieces in TCM Hospital of Kunshan (hereinafter referred to as "our hospital"), so as to provide reference for prescription management and clinical rational drug use of TCM decoction pieces. METHODS: A total of 9 600 prescriptions for TCM decoction pieces in our hospital from Aug. 2017 to Jul. 2018 were randomly selected for prescription review, and the basic rules and problems of the medication were analyzed. RESULTS: Of the 9 600 prescriptions for TCM decoction pieces, the majority of the patients were young people aged＞30-45 (3 904, 40.67％). The prescriptions issued by the department of spleen, stomach and hepatobiliary took the lead (2 301, 23.97％), followed by gynecology and famous old Chinese medicine outpatients. There were many prescriptions (8 105, 84.43％) for 11-20 flavors. There were 3 094 prescriptions (32.23％) with the prescription amount of＞100-200 yuan. There were a total of 8 437 (87.89％) prescriptions for a single dose＞10-40 yuan; 3 552 prescriptions (37.00％) for unreasonable prescriptions, mainly for non-standard prescriptions (3 498 prescriptions). CONCLUSIONS: The unreasonable rate of prescription of TCM decoction pieces in outpatient department of our hospital is relatively high, and some prescriptions have problems such as excessive drug taste, high dose, high amount and irregular prescription writing, which should be attached more importance to ensure the safety and effectiveness of medication.%目的:了解昆山市中医医院(以下简称"我院")门诊中药饮片处方情况及存在的问题,为中药饮片处方管理和临床合理用药提供参考.方法:随机抽取2017年8月至2018年7月我院门诊中药饮片处方9 600张,进行处方点评,分析其用药基本规律及存在的问题.结果:9 600张中药饮片处方中,患者多为＞30～45岁的青年人(3 904张,占40.67％);脾胃肝胆科开具的处方最多(2 301张,占23.97％),其次为妇科、名老中医门诊;开具药味数11～20味的处方较多(8 105张,占84.43％);处方金额＞100～200元的处方共3 094张(占32.23％);单剂金额＞10～40元的处方共8 437张(占87.89％);不合理处方3 552张(占37.00％),主要为不规范处方(3 498张).结论:我院门诊中药饮片处方不合理率较高,部分处方存在用药味数偏多、剂量偏大、金额偏高和处方书写不规范等问题,应予以重视,确保用药安全、有效.
