自攻型微螺钉种植体支抗在口腔正畸中的应用体会

         

摘要

Objective To study on the clinical effect of using the self-drilling miniscrew implant as maximum anchorage in the procedure of orthodontic treatment. Methods Choose 10 patients with maxillary protrusion and being all extracted the maxillary first premolar from 20 patients using the self-drilling miniscrew implant as maximum anchorage for analysis. The site of implantation is the interval near to the buccal side and alveolar bone between the maxillary first premolar and the maxillary first molar by using the self-drilling miniscrew implant as anchorage, and adopting force through nickel titanium tensioned spring and adding its strength once a month, the force is from 150 g to 200 g. To analyze on the lateral cephalometric-ray films before and after the therapy and measure the moving change of central maxillary incisor and the first molar. Results It showed well the maxillary protrusion of 10 patients are improved, the cut edge of the central maxillary incisor has adducted 5.98 mm, the maxillary first molar has moved 1.64 mm, and gets better effect with maximum anchorage. Conclusion The self-drilling miniscrew implant anchorage can provide the maximum anchorage with stable performance, operate simply, the site of implantation is flexible, the patients receive this therapy easily, and it is an ideal anchorage of in the procedure of orthodontic treatment.%目的:研究自攻型微螺钉种植体在正畸治疗过程中作为强支抗的临床应用效果。方法在20例应用自攻型微螺钉种植体作为强支抗的病例中,选出10例上颌前突,均拔除上颌第一前磨牙后采用上颌强支抗进行矫治的患者进行分析。选用自攻型微螺钉种植体作为支抗,植入部位为上颌第一前磨牙与上颌第一磨牙牙根间的颊侧牙槽间隔处,用镍钛拉簧加力,力值150~200 g,每月加力一次。对患者矫治前、矫治后的头颅定位侧位片进行分析,测量上颌中切牙和第一磨牙的移动变化。结果在10例患者中矫治后上颌前突改善明显,上颌中切牙切缘均内收5.98 mm,上颌第一磨牙移动0.64 mm,均获得强支抗的效果。结论自攻型微螺钉种植体支抗可以提供性能稳定强支抗,并且手术操作简单、植入部位灵活、患者易于接受,是正畸治疗过程中理想的支抗选择。
