目的：分析群组管理在社区慢性病管理中的应用效果。方法选取2010年12月至2012年12月期间本院160例慢性疾病患者临床资料，随机将患者分为对照组和实验组，各80例。对照组采取常规的三级管理模式，实验组接受群组管理模式，最后对比两组管理模式的应用效果。结果实验组管理前后收缩压、舒张压、体质量指数变化明显优于对照组，差异显著，具有统计学意义(P<0.05)。实验组患者生活方式行为变化明显优于对照组，差异显著，具有统计学意义(P<0.05)。结论与常规的三级管理模式对比，群组管理有利于改善患者生活行为，控制患者的血压、舒张压、体质量等，提高患者生活质量，具有广泛的应用价值。%Objective To analysis group management in the application of community chronic disease management effect. Methods To choose between December 2010 and December 2010 in our hospital during the period of clinical data of 160 cases of chronic disease patients, patients randomly divided into control group and experimental group, 80 cases each. Control group take the conventional three-level management mode, the experimental group accepted group management mode, finally compare the application effect of two groups of management mode. Results The systolic pressure, diastolic blood pressure, body mass index before and after the management change was better than control group, significant difference, statistically significant (P<0.05). Experimental group was better than control group, experimental group way of life in patients with behavioral changes significant difference, statistically significant (P<0.05). Conclusion Compared with the conventional three-level management mode, is beneficial to improve the patients life behavior, group management control of the patient's blood pressure, diastolic blood pressure, weight and so on, improve patient quality of life, has extensive application value.
