目的 观察七氟烷吸入麻醉在小阴唇粘连分离术中的应用效果.方法 2014年10月至2015年3月在我院择期行小阴唇粘连分离术患儿80例,年龄3个月~6岁,ASAⅠ级,采用随机数字表法分为七氟烷组(S组)和氯胺酮组(K组),每组40例.分别采用面罩吸入七氟烷或静脉注射氯胺酮进行麻醉诱导及维持.记录并比较两组患儿的诱导时间、手术时间、清醒时间以及围麻醉期的不良反应.结果 两组患儿围麻醉期心率(HR)、呼吸频率(RR)和脉搏血氧饱和度(SpO2)均在正常范围,均无呼吸抑制;S组患儿清醒时间明显短于K组[(7.5 ± 2.6) min vs (25.6±9.2) min,P<0.01];S组患儿诱导期兴奋、术中体动及分泌物增加的例数均明显少于K组[6例vs 15例、5例vs 13例、3例vs 18例,P<0.05或0.01).结论 与氯胺酮静脉麻醉比较,七氟烷吸入麻醉诱导迅速,维持平稳,对呼吸循环影响小,苏醒快,不良反应发生率低,适用于小阴唇粘连分离术.%Objective To observe the clinical effect of sevoflurane inhalation anesthesia in children undergo-ing operation for adhesion of labia minora. Methods Eighty children aged 3 months to 6 years old with ASAⅠunder-going operation for adhesion of labia minora in our hospital from October 2014 to March 2015 were divided into the sevoflurane group (group S) and the ketamine group (group K) by random number table, with 40 patients in each group. The anesthesia induction and maintenance were performed respectively with inhalation of sevoflurane by mask in group S or intravenous injection of ketamine in group K. The induction time, operation time, recovery time and the side effects during anesthesia were recorded and compared between the two groups. Results The heart rate (HR), respiratory rate (RR) and pulse oxygen saturation (SpO2) were all in the normal range, and there was no respiratory depression during an-esthesia in two groups. The recovery time in group S was significantly shorter than that in group K [(7.5 ± 2.6) min vs (25.6±9.2) min, P<0.01]. Less patients showed exciting during induction, intraoperative restlessness and increased secre-tion in the respiratory tract in group S than those in group K (6 cases vs 15 cases, 5 cases vs 13 cases, 3 cases vs 18 cas-es, P<0.05 or 0.01). Conclusion Compared with ketamine intravenous anesthesia, sevoflurane inhalation anesthesia can provide a superior anesthetic condition with fast induction, stable maintenance, less depression on respiration and cir-culation, faster recovery, and low incidence of adverse reactions. It is suitable for children undergoing operation for adhe-sion of labia minora.
