单眼弱视者的空间视觉缺损及其与弱视程度的相关性研究

         

摘要

目的:研究单眼弱视者的空间视觉缺损模式及其与弱视程度的相关性,探讨连续全遮盖治疗的临床效果。方法回顾性分析我院2011年8月至2014年4月期间,于我科治疗的65例单眼弱视患者的临床资料,均予以连续全遮盖方式治疗,疗程12周,通过治疗效果,对比患者治疗前后双眼的视力、视锐度、AULCSF (对比敏感度函数的曲线下面积)、Smax (峰值对比敏感度)、Frmax (Smax空间频率)、CutSF (截止空间频率),评价弱视程度对空间视觉缺损所产生的影响。结果经12周治疗后,患者患眼视力、AULCSF、CutSF与治疗前相比较,差异均有统计学意义(P<0.05),患眼Smax、Frmax与治疗前相比较,差异均无统计学意义(P>0.05)。视锐度、AULCSF、Smax、Frmax、CutSF在患眼不同弱视程度下差异均有显著统计学意义(P<0.01),随弱视程度加重,患眼Smax、Frmax、CutSF下降。患眼弱视程度随着空间频率降低,表现为逐渐加重趋势(P<0.05)。结论单眼弱视患者AULCSF、Smax、Frmax、CutSF和视锐度与空间视觉缺损呈一致性趋势。如弱视眼的空间视觉缺损加重,则对应的弱视程度也呈加重表现,如高频端的对比度、分辨率降低,则患眼空间频率也将呈低频率偏移表现。单眼弱视患者连续全遮盖方式治疗不仅可以使视觉高频端的分辨率得到改善,并且不会降低患眼视功能。%Objective To study the space visual defect model of monocular amblyopia and its correlation with the degree of amblyopia, and to discuss the clinical effect of continuous full cover treatment. Methods The clin-ical data of 65 patients with monocular amblyopia admitted in our hospital from Aug. 2011 to Apr. 2014 were retro-spectively analyzed. The patients were give continuous full cover treatment for 12 weeks. The binocular vision, visual sharpness, Area under the curve of the contrast sensitivity function (AULCSF), Smax contrast sensitivity (Peak), Smax space frequency (Frmax), Cut-off spatial frequency (CutSF) before and after treatment were compared to evaluate the effect of degree of amblyopia on space visual defect. Results After 12 weeks of treatment, patients' visual acuity, AULCSF, CutSF showed statistically significant difference with those before treatment (P<0.05), but Smax, Frmax had no statistically significant difference (P>0.05). Visual acuity, AULCSF, Smax, Frmax, CutSF had statistically sig-nificant difference between different degrees of amblyopia (P<0.01), and Smax, Frmax, CutSF decreased with the de-gree of amblyopia eyes increased. Conclusion In patients with monocular amblyopia, AULCSF, Smax, Frmax, CutSF show a trend of consistency with visual sharpness and space visual defects. If the space visual defects deterio-rate, the corresponding degree of amblyopia would get worse. If high frequency contrast and resolution are reduced, the risk of eye spatial frequency will also show low frequency offset. For monocular amblyopia, continuous full cover treatment can improve the visual to the high frequency resolution, and not reduce visual function of eye.
