Objective To study the effect of intra-abdominal hypertension on ventilator weaning failure in COPD patients with respiratory failure. Methods Sixty patients with COPD with respiratory failure using ventilator treatment were divided into 30 cases for the experimental group (intra-abdominal pressurel2~15 mmHg) and 30 cases for the control group (intra-abdominal pressure <12 mmHg). The ventilator weaning failure cases in 24 hours were compared between the two groups. Results There was a statistically significant difference of the rate of ventilator weaning failure between experimental group and the control group. Conclusion The rate of ventilator weaning failure can be reduced by mild intra abdominal hypertension in COPD patients with respiratory failure.%目的 观察腹内高压(IAH)对慢性阻塞性肺病(COPD)呼衰患者脱呼吸机失败的影响.方法 选取COPD呼衰行呼吸机治疗患者60例,按脱呼吸机时腹内压水平分为实验组(腹内压12～15 mmHg)和对照组(腹内压＜ 12 mmHg)各30例,常规试脱机,观察比较24h内脱机失败例数.结果 实验组12例失败(40％),对照组3例失败(10％),实验组与对照组比较差异有统计学意义.结论 COPD呼衰患者脱机时轻度腹内压升高即可以降低脱机成功率.
