目的：主要是探讨脐带血管前置在产前进行超声诊断的临床意义。方法：选择近三年来我院产科收诊60例妊娠合并血管前置的孕妇产前超声检查图像资料并对其结局进行分析。结果：产前超声检测共检测出38例孕妇有血管前置，漏诊2例，检出率为95．0％（38／40），其中，中孕期诊断32例（84．2％，32／38），晚孕期诊断6例（15．79％，6／38）经腹超声检查诊断的30例（78．9％，30／38），其余8例需进一步通过会阴以及阴道超声进行检测（21．0％，8／38）。产前超声确诊的38例孕妇中，其中37例是剖腹产，没有围产期死亡，1例因为其他原因而引产。漏诊的2例，其中1例在经阴道分娩生产时因大出血胎儿死亡，另1例经阴道分娩时大出血，立即剖宫产手术娩出胎儿，经及时抢救，胎儿存活。结论：在孕中期进行脐带血管前置的超声检测是筛查的最佳时期，对于腹部 B 超难以确定的则可以根据情况进行会阴或者阴道的超声检测。总之，产前进行超声检测及时发现脐带血管前置，能有效的降低围产儿的死亡率。%Objective:to primarily explore the umbilical cord blood vessels lead in prenatal ultrasound diagnosis of clinical significance.Methods:se-lect nearly three years the hospital obstetric clinical 60 cases of pregnancy with blood vessels prenatal ultrasound image data and the result is analyzed. Results:prenatal ultrasonic testing were detected 38 cases of pregnant women have a blood vessel in advance,2 cases of misdiagnosis,the detection rate was 95.0% (38/40),among them,the prenatal diagnosis of 32 cases (84.2%,32/38),late pregnancy diagnosis of 6 cases (15.79%,6/38)of abdominal ultrasound examination in the diagnosis of 30 patients (78.9%,30/38),the rest 8 cases need to be further through the perineum and vagi-nal ultrasound examination (21 .0%,8/38).Prenatal ultrasound in the diagnosis of 38 cases of pregnant women,of which 37 cases were cesarean sec-tion,no perinatal death,1 case of induced labor because of other reasons.Missed diagnosis in 2 cases,including 1 case in the vaginal delivery produc-tion because of massive hemorrhage of fetal death,another massive hemorrhage in 1 case after vaginal delivery,cesarean delivery immediately expelling the surgery,the timely rescue,fetal survival.Conclusions:umbilical cord blood vessels in the middle of pregnancy front of ultrasonic testing is the best period,screening for abdominal B to exceed can according to the situation of difficult to determine the perineum or vaginal ultrasonic testing.All in all, prenatal ultrasonic testing umbilical cord blood vessels in front,can effectively reduce perinatal mortality.
