Objective To investigate the clinical efficacy of early mechanical ventilation in the treatment of elderly pa-tients with acute severe organophosphorus pesticide poisoning accompanied by respiratory failure.Methods 46 elderly pa-tients with acute severe organophosphorus pesticide poisoning accompanied by respiratory failure were randomly divided into two groups,23 cases in each group.The control group was treated with conventional symptomatic treatment,and the obser-vation group was treated with early mechanical ventilation in addition.Results All patients survived after emergency,but the rates of poisoning rebound and weakness syndrome in observation group were significantly lower than control group,the difference was statistically significant(P <0.05 ).The serum IL -6 and IL -1 0 values in the two groups were significantly lower than before treatment(P <0.05 ),while the values in treatment group were significantly lower than control group(P <0.05 ).Conclusion Early mechanical ventilation could effectively improve the prognosis in the treatment of elderly pa-tients with acute severe organophosphorus pesticide poisoning accompanied by respiratory failure,and the mechanism may be decreasing the expression of inflammatory cytokines.%目的：观察早期机械通气治疗老年急性重度有机磷农药中毒伴呼吸衰竭患者的效果。方法老年急性重度有机磷农药中毒呼吸衰竭患者46例根据随机数字表原则分为观察组与对照组，各23例。对照组给予常规对症治疗，观察组在此基础上给予早期机械通气治疗。结果所有患者经治疗均存活，观察组中毒反跳和肌无力综合症发生率明显低于对照组，差异有统计学意义（P ＜0．05）。两组的血清 IL －6和 IL －10值均明显降低（P ＜0．05），同时观察组明显低于对照组（P ＜0．05）。结论早期机械通气治疗老年急性重度有机磷农药中毒呼吸衰竭能有效改善预后，其作用的发挥可能与降低炎症因子表达有关。
