The metastases of renal cell carcinoma (RCC) are diverse, common sites of metastases including the lung, liver, bone. Metastases of RCC in paranasal sinus are relatively rare, but really rare in frontal sinus. A patient who had had metastasis in the frontal sinus after radical nephrectomy and immunotherapy, underwent three palliative surgeries for metastasis, radiotherapy and chemotherapy instead of radical resection for metastases or first-line therapy, and the metastasis was uncontrolled. Metastatic carcinomas of the paranasal sinus are most common in the maxillary sinus, and kidney is one of the most common primary site. First-line therapy for clear cell RCC patients with a relapse or distant metastasis includes targeted therapy and immunotherapy.%肾细胞癌(renal cell carcinoma,RCC)转移具有多样性,转移部位常见于肺、肝、骨,转移至鼻窦者相对少见,转移至额窦者则罕见.1例肾癌根治术及免疫治疗后额窦转移的患者未经扩大切除转移灶或一线治疗,行3次姑息手术治疗及放、化疗,鼻窦转移灶未能控制.鼻窦转移癌最常见于上颌窦,肾脏是最常见的原发灶之一.复发或远处转移的肾透明细胞癌患者的一线治疗包括靶向治疗和免疫治疗.
