言语识别率测试在桥小脑角肿瘤诊断中的作用

         

摘要

Objective This study aims at investigating clinical significance of speech recognition score test (SRS) in diagnosis of cerebellopontine angle (CPA) tumors. Methods 19 cases of suspicious diagnosis of CPA tumors received SRS and MRI examination. SRS diagnostic significance was evaluated using MRI as the gold standard in the diagnosis of CPA tumors. Results 19 subjects(20 ears) were recruited, the maximum SRS scores were: 0% in 8 ears, 4% in 2 ears, 8% in 2 ears, 12% in 1 ear, 20% in 1 ear, 40% in 1 ear, 44% in 1 ear, 56% in 1 ear , 68% in 1 ear, 80% in 2 ears. The SRS test rollover effect occur in 7 ears, in which roll -over index (Rl)were 0. 26, 0. 36, 0. 40, 0. 80, 0. 88, 1, 1. The sensitivity and specificity of the maximum SRS score is 75% and 25% , and the sensitivity and specificity of RI were 37. 50% and 75%. 16 cases (20 ears,80%) had abnormal head enhanced MRI imaging results with radiological diagnosis as CPA tumors. Among the 16 cases, 15(15 ears) unden-vent surgical treatment to remove the tumors and the pathological results were as foil wing: 10 were Schwannoma (neurinomas)(66. 67%), 3 were meningiomas(20%), 1 was epidermoid cysts(6. 67%), and 1 was choroid Plexus papillomas(6. 67%). Conclusion SRS plays an important role in diagnosis of CAP tumors. Clinicians should pay more attention to SRS test which should be routined used in clinical audiology.%目的 探讨言语识别率测试(speech recognition score test,SRS)在桥小脑角(cerebellopontine angle,CPA)肿瘤诊断中的作用.方法 对19例(20耳)疑为CPA占位性病变者行SRS测试和MRI检查,并以MRI为诊断CPA的标准评估SRS对CPA肿瘤的诊断意义.结果 19例(20耳)最大言语识别率分别是0%(8耳)、4% (2耳)、8%(2耳)、12%(1耳)、20%(1耳)、40%(1耳)、44%(1耳)、56%(1耳)、68%(1耳)、80%(2耳).SRS测试中出现回跌现象者7耳,回跌指数(rollover index,RI)分别是0.26、0.36、0.40、0.80、0.88、1、1.头部增强MRI示CPA占位性病变者16例(16耳)(16/20,80%),最大言语识别率诊断CPA肿瘤的敏感性为75%,特异性为25%;回跌指数诊断CPA肿瘤的敏感性为37.50%,特异性为75%.接受CPA肿瘤切除术者15例(15耳),病理检查结果分别为神经鞘瘤(听神经瘤)10例(10/15,66.67%)、脑膜瘤3例(3/15,20%)、表皮样囊肿1例(1/15,6.67%)、脉络丛乳头瘤1例(1/15,6.67%).结论 SRS测试有助于诊断桥小脑角占位病变.
