Objective To evaluate the diagnostic value of stereotactic vaccum-assisted breast biopsy( SVAB)on breast atypical ductal hyperplasia( ADH). Methods SVAB with 10-gauge vacuum devices was performed for the 278 patients with non-palpable microcalcification lesions detected by mammography only. Results The biopsy suc-cessful rate was 99. 3%(276/278). Of the 276 patients,27 patients with ADH accounting for 9. 8%(27/276)of total biopsy numbers,included 18 patients with mild ADH,4 patients with midrange ADH and 5 patients with severe ADH. The histological underestimation rates to carcinoma was 0. 0%. Conclusion The histological underestimation rate of SVAB is 0. 0% and it is not necessary for all the ADH to surgical excision.%目的：探讨乳腺X线立体定位真空辅助活检术（ SVAB）在乳腺导管上皮不典型增生（ ADH）诊断中的应用价值。方法278例临床不可触及且超声检查阴性的乳腺微小钙化行SVAB。结果 SVAB活检成功率为99.3%（276/278），276例乳腺微小钙化中，乳腺ADH共27例，占活检总数的9.8%（27/276），其中轻度ADH 18例、中度ADH 4例、重度ADH 5例。未发现组织学低估。结论 SVAB诊断ADH未见组织学低估，且不需要全部进一步手术切检，有较高的临床应用价值。
