目的探讨经皮克氏针内固定与掌锁定钢板内固定治疗成人桡骨远端背侧移位骨折临床疗效。方法选取2012年1月至2015年1月期间收治的123例桡骨远端背侧移位骨折患者，根据其治疗方法分成对照组和观察组。对照组63例采用掌锁定钢板内固定治疗，观察组60例采用经皮克氏针内固定治疗。比较两组患者的手术相关指标，临床疗效以及术后并发症发生率。结果观察组患者手术时间、住院时间及住院花费（32.4±10.2 min，16.2±3.2 d和10.8±2.7万元）明显低于对照组患者（49.8±23.9 min，16.2±3.2 d和1.22±0.3万元）（ P ﹤0.05）；两组患者术后肩、前臂、手功能（ DASH）评分、EuroQol健康指数量评分表（ EQ-5D）评分以及放射学评价指标数据比较差异无统计学意义（ P ﹥0.05）；观察组患者术后并发症发生率（10.0%）明显低于对照组患者（46.0%）（ P ﹤0.05）。结论经皮克氏针内固定治疗成人桡骨远端背侧移位骨折操作简单，创伤小，疗效满意，费用低，值得临床推广应用。%Objective To evaluate the clinical efficacy of Percutaneous fixation with Kirschner wires and volar locking plate fixation in adults with dorsally displaced fracture of distal radius. Methods During January 2012 to January 2015,123 cases of patients with displaced fractures of dis-tal radius were divided into the control group and the observation group ,and there were 63 patients of the control group treated with palm lock plate fixation,60 cases of the observation group treated with percutaneous Kirschner wire fixation. The surgery-related indicators,the clinical efficacy and incidence of postoperative complications of the two groups of patients were compared. Results The operation time,hospitalization time spent in hos-pital opinions of the observation group(32. 4 ± 10. 2 min,16. 2 ± 3. 2 d and 10. 8 ± 2. 7 ten thousand yuan)were significantly lower than those of the control group(49. 8 ± 23. 9 min,16. 2 ± 3. 2 d and 1. 22 ± 0. 3 ten thousand yuan)( P ﹤0. 05);The postoperative DASH score,EQ-5D score and radiological evaluation data of the two group showed no statistically significant( P ﹥0. 05). The postoperative complication rate(10. 0%)of the ob-servation group were significantly lower than those of the patients in the control group(46. 0%)( P ﹤0. 05). Conclusion Percutaneous fixation with Kirschner wires has the advantage of displacement simple,less invasive,satisfactory,low cost for the treatment of adult dorsal distal radius frac-ture,and it is worthy of clinical application.
