目的：观察含莫西沙星抗结核方案在初治涂阳肺结核患者强化期化疗中的疗效。方法临床选取3年间我院收治的符合纳入及排除标准的涂阳初治肺结核患者93例。随机分为观察组和对照组，观察组在强化期给予HRZE常规抗结核方案基础上加用莫西沙星，对照组给予HRZE常规抗结核方案。对强化期各治疗时间点的痰菌阴转情况和其他临床指标进行分析。结果观察组第4、6、8周痰结核菌阴转率明显高于对照组(P<0．05)。胸部CT显示观察组病灶吸收有效率高于对照组(P<0.05)。胸部 CT显示空洞变化有效率高于对照组，但差异无统计学意义。两组药物不良反应相似，无统计学差异。结论含莫西沙星抗结核方案在初治涂阳肺结核患者强化期化疗中可缩短痰菌阴转时间，提高阴转率，提高病灶吸收有效率，不良反应两组类似。%Objective To observe the curative effect of moxifloxacin anti-TB program in the treatment of ini-tial-treated smear positive pulmonary tuberculosis patients during intensive chemotherapy. Methods 93 patients with initial-treated smear positive pulmonary tuberculosis during the past 3 years were included and randomly divided into two groups, the observation group and the control group. The observation group was given routine HRZE regular anti-tuberculosis scheme with a combination of moxifloxacin during the reinforcement phase, and the control group was given routine HRZE regular anti-tuberculosis scheme. The negative conversion rate of sputum bacteria and other clini-cal indicators were compared between the two groups. Results The negative conversion rate was significantly higher in the observation group than in the control group at the 4th, 6th and 8th weeks respectively (P<0. 05). Chest CT images showed the absorption efficiency was higher in the observation group than in the control group ( P<0. 05 ) , and the effective rate of empty hole conversion was better in the observation group than in the control group ( P>0. 05). The adverse reactions of the two groups were similar (P>0. 05). Conclusion Moxifloxacin-containing anti-TB program can reduce sputum negative conversion time and improve the negative conversion rate and lesions absorption efficiency in the treatment of initial-treated smear positive pulmonary tuberculosis patients during intensive chemotherapy, with the similar adverse reactions as the control group.
