Objective To detect the effect of Yadanziyou Oleum Bruceae in the treatment of elderly COPD superinduced therapeutic pneumothorax.Method 136 patients with COPD superinduced therapeutic pneumothorax were randomly divided into 2 groups ( experimental group and control group ) and treated respectively with thrombina and Yadanziyou Oleum Bruceae.Results The total effective rate of control group is 86.8％, the average days in hospital is 14 ±3.3 d, while the total effective rate of experimental group is 95.6％, the average days in hospital is 10 ± 2.5 d ( P ＜ 0.01 ).The chest pain rate of control group is 19.1％, the fever rate is 26.5％,while the chest pain rate of experimental group is 2.9％, the fever rate is 4.4％ ( P ＜ 0.01 ).Conclusion Yadanziyou Oleum Bruceae is more effective in the treatment of elderly COPD superinduced therapeutic pneumothorax.And the days in hospital are shorter and the chest pain rate and fever rate are lower.So it can be popularized in clinic.%目的 探讨鸦胆子油乳胸腔内注射治疗老年COPD并发自发性气胸的治疗效果.方法 将136例老年COPD并发自发性气胸患者随机分为两组,分别向胸腔内注入凝血酶和鸦胆子油乳,观察两组治疗效果.结果 凝血酶组有效率86.8%,平均住院天数14±3.3 d,鸦胆子油乳组有效率95.6%,平均住院天数10±2.5 d,差异有显著性( P <0.01);凝血酶组胸痛发生率19.1%,发热发生率26.5%,鸦胆子油乳组胸痛发生率2.9%,发热发生率4.4%,差异具有显著性( P <0.01).结论 鸦胆子油乳胸腔内注射治疗老年COPD并发自发性气胸比凝血酶有效率高,平均住院天数短,引起的胸痛、发热不良反应发生率低,值得临床推广.
