早期康复训练对急性脑梗死患者血清NGF、NSE、BDNF及运动功能的影响

         

摘要

Objective: To explore the effect of early rehabilitation training on the serum NGF, NSE, BDNF, and motor function in patients with acute cerebral infarction (ACI).Methods: A total of 150 patients with ACI who were admitted in our hospital from October, 2015 to October, 2016 were included in the study and randomized into the observation group and the control group.The patients in the two groups were given anti-coagulation, anti-platelet aggregation, cerebral circulation improving, and brain cell activators.The patients in the control group were given routine neurological nursing, while the patients in the observation group were given early comprehensive rehabilitation training on the basis of stable vital signs and no disease progression within 48 h.The morning fasting peripheral venous blood before treatment, 2 and 4 weeks after treatment in the two groups was collected.The serum NGF, NSE, BDNF, IL-6, hs-CRP, and TNF-α were detected.The activities of daily living and motor function before treatment, 2 and 4 weeks after treatment in the two groups were evaluated.Results: IL-6, hs-CRP, and TNF-α levels 2 and 4 weeks after treatment in the observation group were significantly lower than those in the control group (P<0.05).NGF and BDNF levels 2 and 4 weeks after treatment in the observation group were significantly higher than those in the control group (P<0.05), while NSE level was significantly lower than that in the control group (P<0.05).MBI and FMA scores 2 and 4 weeks after treatment in the observation group were significantly higher than those in the control group (P<0.05).Conclusions: The early rehabilitation training can effectively reduce the inflammatory reaction of nervous system in patients with ACI, regulate NGF, BDNF, and NSE levels, and play the cerebral function remodeling in order to promote the neural function recovery, and improve the motor function.%目的:探讨早期康复训练对急性脑梗死(ACI)患者血清脑源性神经营养因子(BDNF)、神经元特异性烯醇化酶(NSE)、神经生长因子(NGF)及运动功能的影响.方法:选取2015年10月~2016年10月收治的ACI患者150例为研究对象,随机分为观察组和对照组;2组入院后均给予抗凝、抗血小板聚集、改善脑循环、脑细胞活化剂等治疗,对照组给予常规神经内科护理,观察组则在生命体征平稳、病情48h内无进展即进行早期综合康复训练治疗;2组于治疗前及治疗后2周、4周采集清晨空腹外周静脉血,测定血清NGF、NSE、BDNF及白细胞介素-6(IL-6)、超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α),于治疗前及治疗后2周、4周进行日常生活活动能力及运动功能评价.结果:观察组治疗后2周、4周IL-6、hs-CRP、TNF-α水平明显低于同期对照组(P<0.05);观察组治疗后2周、4周NGF、BDNF水平明显高于同期对照组(P<0.05),NSE水平明显低于同期对照组(P<0.05);观察组治疗后2周、4周MBI、FMA评分明显高于同期对照组(P<0.05).结论:早期康复训练可有效减轻ACI患者神经系统炎症反应,调节NGF、BDNF、NSE水平,有效发挥脑的可朔性及功能重建,从而促进神经功能恢复,改善运动功能.
