乌司他丁联合前列地尔对单肺通气患者肺组织氧化应激反应的影响

         

摘要

Objective To observe the effects of ulinastatin combined with alprostadil on oxidative stress in lung tissue of patients undergoing one-lung ventilation. Methods Eighty lung cancer patients scheduled to undergo pulmonary lobectomy were randomly divided into four groups: ulinastatin plus alprostadil group (group U + Q) , alprostadil group (group Q) , ulinastatin group (group U) and control group (group C) 20 cases each. The levels of serum malondialdehyde (MDA) and superoxide dismutase (SOD) were measured immediately after one-lung ventilation (Tl) ,1 hour after one-lung ventilation (T2) and 1 hour after surgery (T3). Results There were no significant differences in serum MDA and SOD levels among the four groups at Tl. Compared with group C,MDA levels significantly decreased and SOD levels obviously increased in the other three groups at T 2 and T 3 (P<0. 05). Moreover,MDA and SOD levels in group U+Q were significantly different from those in group U or group Q(P<0. 05). Conclusion The combined treatment with ulinastatin and alprostadil exerts protective effects on oxidative stress in lung tissue of patients undergoing one-lung ventilation.%目的 观察乌司他丁联合前列地尔对单肺通气患者肺组织氧化应激反应的影响.方法 选择择期全身麻醉下行肺叶切除术的肺癌患者80例.按随机数字表法分为4组:乌司他丁+前列地尔组(U+Q组)、前列地尔组(Q组)、乌司他丁组(U组)、对照组(C组),每组20例.观察单肺通气即刻(T1),单肺通气60 min(T2),术后60 min(T3)的血清丙二醛(MDA)和超氧化物歧化酶(SOD)的浓度.结果 在T1时,各组MDA、SOD浓度差异无统计学意义;在T2、T3时,前3组与C组相比,血清丙二醛(MDA)的水平明显降低(P<0.05),而U+Q组与U、Q组相比,其MDA的水平差异有统计学意义;在T2、T3时,前3组的SOD浓度明显高于C组(P<0.05),而U+Q组与U、Q组相比,SOD的水平差异有统计学意义(P<0.05).结论 联合运用乌司他丁、前列地尔对单肺通气氧化应激损伤有较好的保护作用.
