Objective To explore the relationship between Hyperuricemia(HUA) and insulin resistance(IR) in middle-aged and elderly (age greater than 40). Methods The levels of fasting serum insulin (FIns) were detected by radioimmunoassay with radioimmunity counter,and the levels of serum uric acid(SUA), fasting plasma glucose (FPG), total cholesterol(TC) and triglyceride(TG) were measured by automatic biochemical analyzer in 72 patients with HUA(HUA group) and 64 normal subjects(normal control group). Insulin resistance index(IRI= FPG× FIns/22.5) and insulin sensitivity index[ISI=ln 1/(FPG× FIns)] were counted. Results Compared with the normal control group, the levels of FPG, TG, IRI fasting, FIns and SUA significantly increased,but ISI levels obviously decreased in HUA group (all P＜0.05). There were no significant differences in TC levels between the two groups(P＞0.05). In HUA group,SUA levels were positively correlated with FPG, TG, IRI and FIns levels (r= 0. 509,0. 493,0. 513 and 0. 471 ,respectively;all P＜0.05) ,but negatively correlated with ISI (r=-0.312 ,P＜0.05). Conclusion HUA is closely related to IR in middle-aged and elderly patients,indicating that HUA is a risk factor for the metabolic syndrome.%目的 探讨中老年(年龄＞40岁)高尿酸血症(Hyperuricemia,HUA)与胰岛素抵抗(insulin resistance,IR)的关系.方法 采用放射免疫法,使用放射免疫计数器检测72例HUA患者(HUA组)空腹血清胰岛素(FIns)的水平.采用全自动血生化分析仪检测血清尿酸(SUA)、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)的水平.计算胰岛素抵抗指数(IRI= FPG×FIns/22.5)、胰岛素敏感性指数(ISI)=ln 1/(FPG×FIns).并与64例健康体检者(正常对照组)检测结果 进行比较.结果 与正常对照组比较,HUA组FPG、TG、IRI、FIns、SUA水平均显著升高(均P＜0.05),ISI水平明显降低(P＜0.05).HUA组TC水平与正常对照组比较差异无统计学意义(P＞0.05).HUA组SUA水平与FPG、TG、IRI、FIns水平均呈正相关(r=0.509、0.493、0.513、0.471,均P＜0.05),HUA组SUA水平与ISI水平呈负相关(r=-0.312,P＜0.05).结论 中老年高尿酸血症与IR密切相关,是代谢综合征的危险因素之一.
