鼻咽癌患者同步放化疗后期症状群及影响因素分析

摘要

目的 探讨鼻咽癌患者同步放化疗后期症状群的种类,分析症状群严重程度的影响因素.方法 采用一般资料调查表、M.D安德森症状调查量表(头颈)(MDASI-H&N)、医院焦虑抑郁量表(HADS)对133例鼻咽癌患者于同步放化疗后期进行调查;通过探索性因子分析统计患者症状群的种类,采用多元直线回归分析探索症状群的影响因素.结果 鼻咽癌患者同步放化疗后期存在4个症状群,分别为放疗副反应症状群、情感-疼痛症状群、病感症状群和消化道症状群;年龄、家庭月收入、居住状态、职业状态、宗教信仰、入院时体力状况评分(KPS)、化疗周期、临床分期和抑郁是症状群严重程度的影响因素.结论 医护人员应从症状群的角度对患者进行症状管理,并根据症状群严重程度的影响因素对患者实施个体化干预,在减轻患者症状负担的同时,提高患者生存期生活质量.%Objective To study the symptom clusters and influencing factors for patients with nasopharyngeal carcinoma(NPC) during late concurrent chemo-radiation therapy(CCRT).Methods A total of 133 patients with nasopharyngeal carcinoma during late concurrent chemo-radiation therapy were studied by the general information questionnaire, M.D Anderson symptom inventory of head and neck (MDASI-H&N) and hospital anxiety and depression scale.The exploratory factor analysis(EFA) was conducted to analyze the symptom clusters.The influencing factors of symptoms clusters were studied by multiple linear regressions.Results EFA identified four main symptom clusters, which were adverse radiotherapy reaction symptom clusters, emotion-pain symptom clusters, sickness feeling symptom clusters and the digestive tract symptom clusters.The influencing factors of the severity of symptom clusters included age, monthly income of family, dwelling state, working state, religious faith, KPS of admission, chemotherapy periods, clinical stage and depression.Conclusion Medical staffs should manage patients from the perspective of symptom clusters and carry out individual interventions for patients based on the influencing factors of the severity of symptom clusters to improve the quality of life while alleviating the burden of patient's symptoms.
