上海市CCU护士自觉护理工作量调查及对策探讨

         

摘要

Objective To identify nurses' perception of nursing workload (PNW) and its related factors in the CCU. Methods A self-designed questionnaire was used to survey 216 nurses from CCU in Shanghai. Results Majority (85.2%) of the nurses reported high PNW (of these, 33.8% reported to be burnout). Non-parameter test showed that there were significant differences in PNW among nurses with different ages (x2=16.539,P=0.002), nursing years (x2=13.736,P=0.033), nursing years in current hospital (x2=17.856,P= 0.007) and weekly working hours (χ2=19.621 ,P=0.001). Spearman test found mental pressure and patient number were positively correlated with PNW (r=0.763, P=0.000; r=0.145, P=0.033), and Logistic regression analysis showed that the predicated factors of PNW including nursing years in current hospital, monthly income, mental pressure and weekly working hours (P<0.05). Conclusion Nurses working in CCU have high PNW. More concern should go to younger nurses with higher PNW, and great importance should be attached to establishing an effective nursing human resources allocation system.%目的 了解CCU护士自觉工作量情况,并探讨其相关因素.方法 采用自行设计问卷,调查上海市12家三级综合性医院216名CCU护士的自觉工作量情况.结果 护士中认为工作量非常小和较小为0,14.8%认为工作量适中,51.4%认为工作量较大,33.8%认为工作量超负荷;非参数检验结果表明自觉护理工作量差异有统计学意义的因素为:年龄(χ2=16.539,P=0.002)、工作年限(χ2=13.736,P=0.033)、目前医院工作时间(χ2=17.856,P=0.007)和周上班时间(χ2=19.621,P=0.001);Spearman相关性分析表明精神压力(r=0.763,P=0.000)和患者数(r=0.145,P=0.033)与自觉工作量呈正相关.Logistic回归分析显示影响自觉工作量的主要因素为目前医院工作时间、月收入、精神压力、周上班时间(P<0.05).结论 CCU护士自觉工作量较大,应切实关注年轻护士自觉工作量较大这一问题,并结合CCU特色,缓解护士上班压力,同时引进合理的绩效考核机制,提高护士福利待遇,此外应构建一套系统的适用于我国国情下的CCU护理人力资源配置体系,并开发一套评估人力资源配置效率的工具.
