动力性牵引外固定架结合中药熏洗治疗陈旧性近侧指间关节骨折脱位

摘要

Objective To discuss the clinical outcome of old fracture-dislocation of proximal interphalangeal joint treated with a self-made distraction external fixator which consists of 3 pins and rubbers combined with Chinese herbal. Methods From March 2005 to March 2012, 10 cases of old fracture-dislocation of proximal interphalangeal joint were treated with a pins-rubber distraction external fixator combined with Chinese herbal. Results The follow-up lasted 12 to 60 months, with a mean period of 22 months. All 10 cases of proximal interphalangeal joint healed and reduced completely. The average healing period was 3 months (range, form 2 to 5 months). There was no complication such as infection, pin loosening, and pin breakage. Conclusion A pins-rubber distraction external fixator is a minimally invasive, reliable and effective treatment for old fracture-dislocation of proximal interphalangeal joint. That can reduce joint pain and swelling and promote early recovery of joint function combined with Chinese herbal.%目的：探讨动力性牵引外固定架结合中药熏洗治疗陈旧性近侧指间关节骨折脱位的疗效。方法2005年3月-2012年3月，采用动力性牵引外固定架结合中药熏洗治疗陈旧性近侧指间关节骨折脱位10例。结果本组10例均获随访，随访时间12~60个月，平均22个月。指间关节骨折均完全骨性愈合及关节复位，愈合时间2~5个月，平均3个月。术后无1例发生钢针断裂、松动及针道感染等并发症。结论陈旧性近侧指间关节骨折脱位，应用动力性牵引外固定架治疗具有微创、固定可靠等优点，结合中药熏洗可以减轻局部疼痛及肿胀，促进关节功能早期恢复。
