2型糖尿病与舌鳞状细胞癌的临床相关性分析

摘要

目的:探讨2型糖尿病与舌鳞癌发生发展及预后的相关性.方法:分析187例初诊为舌鳞癌患者的临床资料,其中合并2型糖尿病者31例,未合并糖尿病者156例,对比分析两组年龄、性别、空腹血糖、临床分期、病理分级、颈淋巴清扫手术率、淋巴结转移率、肿瘤复发率、术后并发症、平均住院天数.结果:舌鳞癌患者中2型糖尿病患病率是16.6％;糖尿病组及非糖尿病组舌癌晚期患者比例分别为71.0％和44.9％(P＜0.05);糖尿病组与非糖尿病组病理分级、颈淋巴清扫手术率和淋巴结转移率差异无统计学意义(P＞0.05);糖尿病组肿瘤复发率(32.3％)和术后并发症(16.1％)明显高于非糖尿病组(13.5％和4.5％)(P＜0.05).结论:合并2型糖尿病的舌鳞癌患者初诊时分期较晚,复发率较高.%Objective:To investigate the correlation of type 2 diabetes mellitus (DM) with tongue squamous cell carcinoma(TSCC).Methods:187 patients with TSCC were retrospectively analyzed.31 cases with DM and 156 without DM were included in DM group and Non-DM group respectively.The age,gender,fasting plasma glucose,clinical stage,pathological grade,neck dissection surgery rate,metastatic lymph node rate,tumor recurrence rate,postoperative complications and average number of days of hospitalization of the 2 groups were compared and analyzed.Results:The prevalence of type 2 diabetes mellitus was 16.6％ in patients with TSCC.The proportion of the patients with the clinical stage Ⅲ and Ⅳ in DM group and Non-DM group were 71.0％ and 44.9％ respectively (P ＜ 0.05).There was no significant difference in terms of pathological grades,neck dissection surgery rate and metastatic lymph node rate between the 2 groups (P ＞ 0.05).The recurrence rate and postoperative complications of DM group (32.3 ％ and 16.1％) were higher than those of Non-DM group(13.5％ and 4.5％).Conclusion:Type 2 diabetes mellitus may positively correlate with late clinical stage,high recurrence rate of TSCC.
