目的：分析7年来对危重患者行连续性肾替代治疗的临床资料，观察急性肾衰竭及急性胰腺炎患者早期行 RRT的治疗效果及预后分析。方法入选患者515例，其中急性肾衰286例，急性胰腺炎116例。对早期及非早期RRT患者的临床预后进行统计。结果单纯急性肾衰竭患者98例，急性肾衰合并多脏器功能衰竭患者177例，两组患者的死亡率有明显差别，对出现急性肾衰者，早期行RRT的患者比非早期行RRT患者预后好；对急性胰腺炎患者，早期行 RRT比非早期行RRT的患者预后好。结论对急性肾衰竭和急性重症胰腺炎患者，早期RRT比非早期RRT，其死亡率明显降低；对危重患者早期行RRT在疗效及预后上有显著意义。%Objective To analyze the dates for citical y il patients in 7 years,which were threapied with continuous renal replacement therapy (CRRT) ;and to observe the therapeutic ef icancy and the prognosis of early RRT for Acute Renal Faliure(ARFs) and Acute Pancreatitis(APs). Method 515 patients,of which,286 with ARF and 116 with AP. To discover statistical information on the prognosis of patients treatede with early RRT and non-early RRT. Results The mortality of Patients with ARF (98) were significantly dif erent from complicated with multiple organ failure (MOF)(117).The ARFs,who treated by early RRT had bet er prognosis than non-early RRTs;The same way,the APs,who treated by early RRT had bet er prognosis than non-early. Conclusions Early RRT contributing to the poor mortality in ARFs and APs,furthermore,there was significant dif erence on the therapeutic ef icancy and prognosis of the citical y il patients.
