弥散张量面神经成像技术在术前预测听神经瘤与面神经关系应用的研究进展

         

摘要

As a common intracranial benign tumor,acoustic neuroma is located in the internal auditory canal and cerebellopontine angle,and facial paralysis is a serious complication of surgical treatment of acoustic neuroma.Diffusion tensor imaging (DTI) is an emerging noninvasive magnetic resonance technique.DTI facial nerve reconstruction can reconstruct the spatial position of acoustic neuroma and facial nerve before surgery,so that the surgeon can understand the course of facial nerve before surgery and achieve maximum removal of the tumor with good facial nerve function reserved.Now,different research centers obtained positions of facial nerve by DTI before surgery do not coincide with the intraoperative neurophysiological monitoring,so combination of the two methods is more beneficial to reduce intraoperative facial nerve injury.%听神经瘤是位于内听道及桥小脑角区域的颅内常见良性肿瘤,面瘫是听神经瘤手术治疗的严重并发症之一.弥散张量成像(DTI)是一种新兴的非侵入性磁共振检查技术,DTI面神经重建技术可在术前重建出听神经瘤与面神经的空间位置关系,使术者在术前能够了解面神经的走形,以达到最大限度切除肿瘤、保留患者面神经功能的目的.但目前各个研究中心得到的DTI术前重建面神经位置与术中神经电生理监测定位吻合率不一样,因此术中需结合神经电生理监测以减少面神经损伤.
