目的：探讨宫颈癌根治术中加用顺铂的临床效果。方法选择2004年3月至2009年3月建湖县人民医院收治的须行宫颈癌根治术的患者300例，根据术中是否加用顺铂治疗分为顺铂组和对照组，各150例。随访5年，观察两组患者术后复发率、生活质量，对比病死率，分析顺铂诱导Caski细胞凋亡的机制。结果对照组患者5年复发率、病死率高达64.0％（96／150）和34.7％（52／150），顺铂组分别为29.3％（44／150）和20.7％（31／150），差异有统计学意义（P＜0.05）。治疗后顺铂组患者 Kupperman 分数低于治疗前[（16.6±6.5）分比（20.4±3.7）分]，但明显高于对照组[（6.2±2.4）分]，差异有统计学意义（P＜0.01）。顺铂组患者性生活、婚姻质量等明显高于对照组，差异有统计学意义（ P＜0.01）。结论宫颈癌根治术术中加用顺铂可降低5年复发率和病死率，提高患者生活质量，在利用顺铂诱导 Caski细胞凋亡的同时给患者带来好处，需完善治疗指征后广泛推广。%Objective To investigate the effect of cisplatin in radical resection of cervical cancer. Methods Total of 300 cases with cervical cancer admitted to Jianhu People′s Hospital from Mar.2004 to Mar.2009 were divided into cisplatin group and control group according to whether with cisplatin was applied in the surgery,150 each.Follow-up of 5 years was done to observe the quality of life,recurrence rate,survival rate of the two groups,and the mechanisms of cisplatin inducing Caski cell apoptosis were analyzed.Results As time progressed after the operation,the recurrence rate and fatality rate of both groups were increased, but the control group was rising faster,with the 5-year recurrence rate and mortality rate as high as 64.0%(96/150 ) and 34.7%( 52/150 ) , which of the cisplatin group was respectively 29.3%( 44/150 ) and 20.7%(31/150),the differences were statistically significant(P<0.05).After treatment,the Kupperman score of the cisplatin group were lower than those before treatment[(16.6 ±6.5) scores vs (20.4 ±3.7) scores] ,but were significantly higher than those in the control group[ ( 6.2 ±2.4) scores] ,the difference was statistically significant(P<0.01).The sex life and marriage quality of the cisplatin group were signifi-cantly higher than those in the control group, the difference was statistically significant ( P <0.01 ) . Conclusion Cisplatin can obviously improve the 5 year survival rate of patients with radical resection of cer-vical cancer,reduce the recurrence rate,improve the life quality of the patients,and bring benefits to patients while inducing Caski cells apoptosis by cisplatin ,,thus should be widely promoted after improving the treat-ment indication.
