袋鼠式护理缓解足跟采血所致新生儿疼痛的Meta分析

         

摘要

目的:应用Meta分析的方法对袋鼠式护理缓解足跟采血所致新生儿疼痛的效果进行分析,为新生儿疼痛管理提供循证依据.方法:在PubMed、Medline、Web of Science、CNKI、CBM、万方数据库中检索相关文献;对纳入文献进行质量评价并采用RevMan5.3进行Meta分析.结果:共纳入5项研究,Meta分析结果显示袋鼠式护理组患儿心率在采血期[WMD=-8.82,95%CI(-11.45,-6.20),P<0.01]及恢复期[WMD=-9.34,95%CI(-12.16,6.53),P<0.01]均低于照组.袋鼠式护理组血氧饱和度在采血期高于对照组[WMD=3.13,95%CI(1.76,4.51),P<0.05],恢复期也高于对照组[WMD=2.62,95%CI(1.48,3.78),P<0.01].袋鼠式护理组采血期啼哭时间短于对照[WMD=-31.83,95%CI(-58.51,-5.17),P<0.05].1项研究报道了疼痛量表(PIPP)评分,袋鼠式护理组PIPP评分低于对照组[WMD=5.30,95%CI(-7.53,-3.07)].1项研究报道了疼痛量表(NFCS)评分,袋鼠式护理组采血期NFCS评分低于对照组[WMD=-0.81,95%CI(-1.52,-0.10)].结论:袋鼠式护理与传统护理方式相比,利于稳定新生儿足跟采血过程中心率,缓解血氧饱和度的下降,缩短啼哭时间,减缓新生儿疼痛.%Objective: To analyze the effect of kangaroo care in alleviation of the pain caused by heel blood sampling of the newborns , so as to provide evi-dence-based basis for pain management of the newborns. Methods: The related literatures were retrieved in PubMed, Medline, Web of Science, CNKI, CBM, and Wanfang database;and quality evaluation was conducted to the included literatures , and also Meta analysis was conducted to them with RevMan 5.3.Results: Five studies were included in total, and Meta analysis results showed that, the child patients from the kangaroo care group had a lower heart rate than those from the control group in the blood sampling period [WMD=-8.829,5 %CI( -11.45,-6.20),P<0.01]and the recovery period[WMD=-9.34,95%CI( -12.16,6.53),P<0.01].The child patients from the kangaroo care group had a higher oxyhemoglobin saturation than those from the control group both in the blood sampling period [WMD=3.13,95%CI(1.76,4.51),P<0.05]and the recovery period[WMD=2.62,95%CI(1.48, 3 .78 ) ,P<0 .01 ] .The child patients from the kangaroo care group had a shorter crying time than those from the control group in the blood sampling period [WMD=-31.83,95%CI( -58.51,-5.17),P<0.05].It was reported by a study that the kangaroo care group was inferior to the control group in sco-ring of premature infant pain profile(PIPP )[WMD=5.30,95%CI( -7.53,-3.07)].It was reported by a study that the kangaroo care group was inferi-or to the control group in scoring of neonatal facial coding system(NFCS) in blood sampling period[WMD=-0.81,95%CI( -1.52,-0.10)].Conclu-sion: Compared with traditional nursing method, kangaroo care could help to stabilize the newborn's heart rate in the process of heel blood sampling, ease the decline of his/her oxyhemoglobin saturation, shorten his/her crying time, and also alleviate his/her pain.
